PATIENT CARE COORDINATOR

Part-Time 11-22 hours pw

Starting Salary: £20,007 pro rata

An exciting opportunity has arisen within AshLea Medical Practice for a part-time Patient Care Coordinator to join our Reception Team at Gilbert House and Linden House Surgeries.

We are a 2 site practice in Ashtead and Leatherhead in Surrey, with 10 partners and approximately 60 staff, our multi-disciplinary team includes Associate GPs, GPwSIs, Clinical Pharmacists, Physiotherapists and a multi-skilled Practice Nurse team.

We serve approximately 21,000 patients. We have close links and are actively involved in the Leatherhead Primary Care Network and GP Health Partners GP Federation.

We are seeking an enthusiastic and committed individual to join our reception team in supporting the practice clinical team signposting patients to the appropriate healthcare professional or service and working efficiently as part of the practice’s multidisciplinary team. The Patient Care Coordinator will undertake reception duties as part of the role, delivering a polite and professional service, ensuring high standards of data recording and fulfilling all administrative tasks to ensure that the Practice is operating as an effective organisation. The Practice will actively support the successful candidate’s personal development and offers career progression.

The following are the core responsibilities of the Patient Care Coordinator for which full training will be given:

a. Process and effectively signpost patients to the appropriate healthcare professional depending on the presenting condition.

b. Recognise and respond to emergency situations.

c. Answer incoming phone calls, transferring calls or dealing with the callers’ requests appropriately and redirecting them to the practice website where possible.

d. Process patient requests for appointments, information and take payments for fee paying services.

e. Process repeat prescription requests.

f. Initiate contact with and respond to requests from patients, team members and external agencies.

g. Promote and process patient online access and proxy access via the NHS App.

h. Action incoming emails and tasks.

i. Code patient data as required on EMIS Web.

j. Photocopy/scan/workflow documentation as required.

k. Data entry of new patient registrations and processing the relevant patient registration information as required. Process registration queries.

l. Input data into the patients’ healthcare records as necessary.

m. Deal with requests for information, e.g. SAR, insurance/solicitors’ letters and DVLA forms, reviewing the patient records for updates. If unavailable direct patient to the administrative team.

n. Manage all queries as necessary in an efficient manner.

o. Endeavour to resolve all patient concerns. However, if unable to find a resolution forward for the Patient Services Manager’s attention.

p. Carry out system searches as requested.

q. Maintain a clean, tidy, effective working area including waiting room & kitchen at all times.

r. Monitor and maintain the reception area and noticeboards.

s. Assist to ensure all specimens collected by courier.

t. Clean up spills/accidents as soon as practicable in accordance with practice policy.

u. Complete opening and closing procedures in accordance with practice policy.

v. Support all clinical and management staff with general tasks as requested.

In addition to the primary responsibilities, the Patient Care Coordinator may be requested to:

a. Keep notes tidy and in alphabetical order and putting to one side any notes that need repairing, pruning or summarising. Pull notes as required (Gilbert House).

b. Participate in practice audit as directed by the audit lead.

c. Support the care coordinating team with administrative staff, providing cover during staff absences.

d. Complete opening and closing procedures in accordance with the duty rota.

e. As required, support the partners, Associate GPs and other prescribers in the management of repeat prescriptions, ensuring that they are processed accurately and efficiently.

f. Order and monitor stationery supplies in Patient Services Manager’s absence.
