We are a short-term service designed to help people to develop confidence and skills they need to live as independently as they can in the community. The number of calls that individuals receive are continuously monitored and reduce as they regain their independence.

What is Reablement?

It’s supporting our service users to regain their confidence & independence in their own homes.

It’s about helping people to do things themselves; not doing it for them.

What do we do?

We will develop an individual’s programme with them and provide the support they need to be the best they can be.

We show our service users ways to help themselves and include (with their permission) their care/family in all relevant discussions.

The Reablement team liaise with several other key service providers, including Occupational Therapists, Physios and Social Workers - all working with the service users best interests at heart, to gain the confidence and skills to be comfortable living independently at home.

As a member of the Reablement team, your voice is heard; and your observations from your visits to service users homes are included as part of the overall care assessment of that individual.

Your next role

As a Support Worker, you will work as part of a team who were awarded HIGHLY COMMENDED by the Health Service Journal awards and we were also finalist for Medical Journal award for improving value in the care of frail older patients and integrated discharge planning within Swindon.

Your responsibilities will include personal care (e.g. washing, dressing, toileting); food and drink preparation; mobility and transfers; medication prompt; and working with other Health and Social care professionals within the Multidisciplinary approach to achieve the best outcome for individuals we support.

You must be fluent in the English language (as a requirement of Part 7 of the Immigration Act “for the effective performance of a customer-facing role”) and a hold a full driving licence and access to a car.

Working with us

We offer a mixture of shift patterns (18 to 36 hours) consisting of 6 hour shifts from 7:00am to 1:00pm and 4:30pm to 10:30pm across 7 days.

Your tax free mileage is paid at 45p/mile, which is the top government allowance.

Your roles is salaried where overtime is optional and never required.
