Due to career progression Pembrokeshire County Council Reablement Team have a vacancy for Co-ordinator. The post holder will work in our small friendly team which delivers person centred reablement support to customers within the community and will take on a co-ordination role within the service. You will be working alongside Team Leaders OT’s and the Registered Manager, ensuring the smooth running of the runs and rotas to ensure delivery of care is maintained. This will involve managing and maintaining the electronic scheduling system ensuring customer visits are scheduled, carers are assigned appropriately and timings are optimised ensuring both value for money and delivery of high standard of support. You will need excellent communication skills as you will be liaising with Health and Social Care Professionals as well as the individuals we support and their families. You will also be required to work closely with our Domiciliary colleagues when scheduling the service.
Ideally you will need to hold or be working towards a Level 3 qualification in a relevant discipline, for example business administration and have experience of working within a business or care environment.
