As a Registered Nurse, the post holder will work within the Right Start and School Nursing Service to deliver the Healthy Child Programme 5-19.
Working within the School Nursing Team, the post holder provides a comprehensive service to meet the needs of children, young people and their families, applying the principles of positive health and wellbeing, the prevention of ill health with individuals, families and the wider community.
We have various hours available, permanent and fixed term contracts.
Main duties of the job
To support delivery of the Healthy Child Programme 5-19 using evidence-based practice to actively promote the health of children and young people.
To deliver and engage in health promotion programmes and public health initiatives to prevent ill health, early identification of abnormalities and work with individuals, Schools and communities to reduce health inequalities as identified within individual School Health Plans.
To conduct health needs assessments in a planned programme of care within a variety of settings. Offer appropriate interventions, delivering agreed programmes of care and evaluating the outcomes for the child young person and family.
Undertake comprehensive health assessments for identified children and young people, including annual health assessments for those in local authority care in line with the Healthy Child Programme 4-19 years of age.

Detailed job description and main responsibilities

Establish clear and effective communication links with children and young
people, parents/carers, education staff, and a range of other professionals
from both voluntary and statutory organisations.
Communicate outcomes of assessment and future care planning to the named School Nurse.
Where necessary carry out home visits to children and families advising on all aspects of health as appropriate.
Participate in health needs assessment of the local community, contributing to the planning, implementation, and evaluation of strategies to address unmet health needs.
To identify and assess children 5-19 with complex health needs, offering an enhanced integrated service within the multi-agency arena.
To identify, assess, report, and work independently with oversight from the School Nurse to safeguard and protect the health and wellbeing of children and young people working within safeguarding policies and guidelines. To attend Child Protection case conferences and other meetings as appropriate.
To provide on-going support for children and young people in care including the statutory health assessment and on-going review.
Listen appropriately and take into account any sensitive issues and communicate these to appropriate persons/agencies ensuring that children are protected and that disclosures are acted upon appropriately.
Feedback the views and needs of children/young people, families and schools to the School Nurse as necessary, to enable appropriate interventions to be implemented.
To work in collaboration with other agencies to deliver services to children, young people and their families

Experience/Skills/Knowledge
Essential criteria
- Registered Nurse with NMC - current registration. Evidence of recent professional development Mentorship (willing to work towards)
- Able to demonstrate evidence based clinical practice Experience of working with children, young people and their families Experience of Multi agency working
- Able to demonstrate knowledge of safeguarding procedures and legislation for children and young people Able to work as an effective team member Ability to work independently
- The ability to travel independently throughout Oldham. The ability to work flexibly in accordance with service needs
Desirable criteria
- Experience of delivering childhood immunisation programme Working within a community setting Experience of using spread-sheets and Databases LEO or equivalent
- Knowledge of the Healthy Child Programme Knowledge of current Public Health Outcomes and inequalities
