Job Summary
The Qualified Intellectual Disabilities Professional (QIDP)/Program Supervisor is responsible for integrating, coordinating and monitoring the active treatment process for 6 intellectually disabled adult residents residing in a long-term care Intermediate Care Facility for the Developmentally Disabled Habilitative (ICF-DD/H), licensed by the California Department of Public Health (CDPH).
Serra Center’s ICF-DD/H Homes are designed to provide 24-hour care, supervision and support to individuals who have a recurring but intermittent need for skilled nursing services.
With 48 years of experience and community relationships, Serra Center’s dedicated team of nurses, physicians and program professionals work directly with each individual and their family to develop a personal care plan in a residential home setting that focuses on each individual’s medical and support care needs, as well as facilitating individual choice, goals and dreams.
Responsibilities and Duties
The QIDP/Program Supervisor oversees the daily management and operation of 3 residential care ICF-DD/H group homes in Fremont, CA. The QIDP is responsible for the care planning and over-site for a total of 15 residents and direct supervision of 12-16 Direct Support Professionals. This is a full-time, exempt, professional management position with benefits.
Qualified candidates must meet the United States education equivalency requirements and be cleared by the Department of Developmental Services (DDS) in order to be considered for the QIDP position:
- At least one year of experience working directly with persons with intellectual disabilities or other developmental disabilities.
- A human services professional possessing at least a bachelor's degree in a human services field, including but not limited to sociology, special education, rehabilitation counseling or psychology, or is either a physician and surgeon, or an osteopathy physician and surgeon, or a registered nurse.
Qualifications and Skills
- Prior experience with developing and managing active treatment and behavior modification
- Management/supervisory experience at the program level
- Knowledge and experience of Title 22 of the California Health and Safety Code for ICF-DD/H facilities
- Experience and knowledge of the Licensing and Certification process
- Experience with being on-call; serve as back-up to the on-call scheduler every-other-week
- Ability to adapt to change, multi-task, prioritize and meet deadlines
- Willingness to work in a team-oriented environment
- Excellent written and verbal communication skills
- DOJ criminal clearance
- Pre-Employment Physical and TB Test
- Valid CA Driver’s License, Social Security Card and Proof of Vehicle Insurance
