A great opportunity has arisen for an enthusiastic and motivated specialist physiotherapist to join our Discharge to Assess/Supported Discharge Team based at Furness General Hospital.
The role is community based and covers a large geographical area around Barrow in Furness. The team forms part of the Cross Bay Supported Discharge Services, with teams located in Furness, Lancaster and South Lakes. Our team is part of the University of Morecambe Bay Trust, situated close to the Lake District with easy access to large cities such as Preston and Manchester via the M6.
You will work alongside Occupational Therapists, Physiotherapists, Technical Instructors and Clinical Support Workers in the Furness Discharge to Assess and Supported Discharge team. You will be responsible for the support and supervision of technical instructors, with opportunities to develop your role as a clinical educator and taking responsibility for undergraduate students. You will also be encouraged and supported to engage in, and lead training as required. Good communication and team working skills are essential for this post.
This post does carry a requirement for weekend/ on call working as part of current services agreements. These may be subject to change due to requirements to deliver a SDT/D2A 7 day service.
Full and Part time working patterns will be considered for this role.
Your role will include providing physiotherapy treatment to patients on the supported discharge caseload, include assessing and supporting patients discharged home/to care homes on the home first pathways.
Your role in based in Furness, however you may be at times be required to cover the wider geographical area covered by Morecambe Bay including North Lancashire, South lakes and Furness.
You will provide specialist physiotherapeutic assessment, diagnosis, treatment and advice for a range of frequently complex conditions for patients on their caseload.
To be responsible for your own clinical record keeping, including hand-written and electronic data recording, in accordance with Trust, Care Group and Chartered Society of Physiotherapists standards.
To take an active part in In-Service Training and external education in line with Continual Professional Development (CPD) Care Group Guidelines, to maintain standards of care in line with evidence based practice. To disseminate information from training attended as required.
You will be part of a very friendly and welcoming team who support each other and work very closely to provide the best care for our patients in the Morecambe Bay area.
We operate from three main hospitals - Furness General Hospital (FGH) in Barrow, the Royal Lancaster Infirmary (RLI), and Westmorland General Hospital (WGH) in Kendal, as well as a number of community health care premises including Millom Hospital and GP Practice, Queen Victoria Hospital in Morecambe, and Ulverston Community Health Centre.
FGH and the RLI have a range of General Hospital services, with full Emergency Departments, Critical/Coronary Care units and various Consultant-led services.
WGH provides a range of General Hospital services, together with an Urgent Treatment Centre, that can help with a range of non-life threatening conditions such as broken bones and minor illnesses.
All three main hospitals provide a range of planned care including outpatients, diagnostics, therapies, day case and inpatient surgery. In addition, a range of local outreach services and diagnostics are provided from community facilities across Morecambe Bay.
Our community services for adults are provided in people’s homes, community centres, clinics, GP Practices, community hospitals and our main hospitals. Our aim is to work with people to help them remain independent, improve their health and manage their conditions through high quality care, advice and support.
