About the Role
The successful candidate will become an integral part of our multidisciplinary team, with mentorship provided by the GP team and will work to demonstrate improvement in patient outcomes and in the efficiency of the practice, delivering the highest standard of service.
The role will include the following duties:
- Care home ward rounds.
- Individual care planning.
- Acute response services.
- Running of clinics providing care for all presenting patients, from initial history taking and clinical assessment through to the diagnosis, treatment and evaluation of care.
- Providing an holistic clinical service, by working collaboratively with the healthcare team and with support from GPs as required.
- Meeting the needs of the patients, implementing agreed management plans and following approved protocols as appropriate.
- Carrying out home visits and consulting with patients who present on the day, via booked appointments.
- Reviewing and acting appropriately on blood results, referrals and correspondence regarding patients.
About the Candidate
- A postgraduate degree in Physician Associate Studies from an accredited PA programme.
- Membership of the Faculty of Physician Associates at the RCP, which includes UK Physician Associate Registration (the PA Managed Voluntary Register).
- Current and valid certification or re-certification by the UK Physician Associate National Examination.
- Experience including preventative medicine and chronic disease management.
- Flexible and adaptable approach to a varied and high-volume workload, with the ability to multi-task and to work under pressure.
- Consistently compassionate and respectful approach, placing a high value on the dignity of patients, relatives, carers and colleagues.
- Ability to communicate effectively with colleagues, patients, relatives, nurses, other staff and agencies.
- Commitment to continuing professional development.
