Working Full Time or Part Time
We are looking for a Rehab Assistant to work within the Rapid Response Team, providing an early supported discharge service to patients from The Countess of Chester Hospital & Ellesmere Port Hospital.
You will be required to work shifts over 7 days between the hours of 8am and 9pm.
You will be lone working across the West Cheshire Community.
The post involves assisting patients with their activities of daily living, providing resettlement, rehabilitation & care to patients in their own home, delivered in accordance to the patient’s Care Plan.
The post holder will support therapists and nurses with their patient caseload, will manage a caseload of your own under the supervision of qualified staff & Senior Community Support Workers, and have general administrative and housekeeping tasks as required to facilitate the smooth running of the service.
The post holder must have excellent interpersonal skills, be self-motivated, empathetic and able to develop practical skills. It is essential to have a holistic view to patient care, with focus on facilitating patient safety & independence in their own home.
The Rapid Response Team provides a service 7 days per week including bank holidays and evening shifts.
The Countess of Chester Hospital NHS Foundation Trust consists of a 600 bedded large District General Hospital, which provides its services on the Countess of Chester Health Park, and a 64 bedded Intermediate Care Service at Ellesmere Port Hospital.

Please note applicants will be required to pay for their DBS check. Costs are deducted from salary following appointment. The cost of the DBS application is £23 (standard) or £43 (enhanced), this cost will be deducted from your salary over the first three months of employment.
