Job description
We are looking for a dynamic and enthusiastic Physical Health Care Assistant to support the physical health needs of patients with Serious Mental Illness (SMI) in the community.
This Vacancy/JD is currently subject to Job Evaluation/Banding.
People with Severe Mental Illness (SMI) suffer from disproportionate physical health conditions compared to people from a similar age and background without SMI.
This post will complete the PH monitoring, checks and interventions for this patient group across Primary and Secondary Care. The checks will be carried out in Team bases and patient homes.

What we want from you!
- Previous experience working in a care or rehabilitation setting
- Holistic approach to care.
- Ability to work alone with patients.
- Ability to work in a variety of settings
- Good level of secondary education with demonstrable literacy and numeracy skills.
- Experience of using Office computer programs including Excel, Word and Outlook
- Good time management skills and ability to prioritise work
