Practice Pharmacy Technician
Hours
Full time – 37.5 hours per week
Accountable to
Clinical Pharmacist
Job Purpose:
To help achieve efficiencies and effectiveness within the prescribing activities of the practice.
To promote high quality, cost effective and evidence based use of medicines within Primary care.
To work under the management under the management of the clinical pharmacist to ensure a safe and efficient service is maintained, delivering a range of medicines management activities including medicines reconciliation.
Main Duties:
The post holder will:
Support the role of the clinical pharmacist in the delivery of the clinical pharmacy services.
Perform medicines management improvement work to maximise safe, cost effective best practice in prescribing under the supervision of the GP or clinical pharmacist.
To be part of the team providing a first point of contact for patients and community pharmacies on medicines management issues. Providing information to patients and/or carers to ensure they are fully informed of any prescribing changes. To help resolve issues, or to escalate then to the practice pharmacist.
To contribute to the discussion and formulation of practice based projects and clinical audits to improve medicines management performance, improving medicines safety and reducing medicines waste.
To work with the practice pharmacist to support the practice to achieve their quality and cost improvement targets.
To carry out medicines optimisation tasks including effective medicine administration, supporting medication reviews and medicines reconciliation post – discharge and other transfer of care.
To provide patient education under the direction of the practice pharmacist.
To be responsible for the development of own professional knowledge and competence and ensure mandatory training is up to date.
To be able to use and willing to learn the GP practice computer system, ensuring all information entered/retrieved is accurate. Also familiar with Microsoft word and excel.
To keep abreast of new developments and attend training courses where appropriate.
