Job Title: PCN Pharmacy Technician

Place of work: Remote

Job Role & Purpose

Pharmacy technicians play an increasingly important role in complementing clinical pharmacists, and multi-disciplinary teams in Primary Care Networks (PCN). The role of the technician will cover clinical, technical and administrative duties. The primary purpose of this role is to ensure that systems, structures, and processes are in place to deliver the smooth and efficient day to day running of pharmacy services across the sites of the PCN.

This involves leading systems to contribute to safe and cost effective, best practice.

The successful candidate will be directly responsible for supporting clinical pharmacists and their teams, along with admin and prescribing teams to ensure we deliver excellent quality clinical and non-clinical services and to deliver optimal care for patients and service users.

Main Duties:

Support the role of PCN Clinical Pharmacists in the delivery of clinical pharmacy services.

Perform medicines improvement work to maximise safe, cost-effective best practice in prescribing under the supervision of the Clinical Pharmacist/s Assist with medication safety monitoring systems (e.g. high-risk drugs)

Liaise with clinicians and administrative staff to resolve medicine-related queries

Monitor practice prescribing and adherence to the PCN’s, included but not limited to any other local and national prescribing policies and guidelines

Improve service and quality through mechanisms such as audits

Support the Clinical Pharmacists in Structured Medication Reviews (e.g. organise necessary monitoring tests before SMR

Undertake patient-supporting roles to ensure effective medicines use through shared decision-making

Support medicine reconciliation of patients whose care is transferred back into

primary care in a timely and effective manner liaising with clinical and non-clinical staff along with patients and other providers to ensure patients receive appropriate medication post discharge

Complete discharge letters for all members practices when required

Carry out medicines optimisation tasks including effective medicine administration, supporting medication reviews and medicines reconciliation on transfer of care.

Assist Clinical Pharmacists with any searches, data collection, medicine safety tasks.

Increase uptake of Electronic Repeat Dispensing and the use of the Electronic Prescription Service.

Support the PCN to deliver on QOF and locally commissioned enhanced services

Implement changes to medication that result from CAS/MHRA alerts, product withdrawal and other local/national guidance.

Work collaboratively within the whole PCN team.

Develop relationships and work closely with community, hospital and MMT pharmacy colleagues and the wider health ecosystem

Complete all required training before going live as advised by the PCN

Maintain GPHC registration and complete all required CPD and any other mandatory requirements to maintain GPHC registration

Attend and complete all required training on the required days to meet PCN requirements

Participate in practice and meetings which consist of but not limited to weekly internal meetings, supervisor meetings, training meetings, clinical governance meetings

Partake in the development of SOP’s and protocols as advised by the PCN

Work with and train PCN personnel along with admin and prescribing teams for each member practice when required

Participate in practice meetings, patient participation groups and other local meetings to improve engagement with the role of pharmacy technician

Complete all reports, tasks or any other work needed to deliver an excellent pharmacy service as identified by the PCN

Work within own scope of competence at all times

Person Specification – Essential requirements:

Registration with the GPhC

BTEC/NVQ level 3 or equivalent in pharmaceutical science/pharmacy services Evidence of continued professional development

Experience of working within a community or hospital setting

Has a planned and organised approach with an ability to prioritise their own

workload to meet strict deadlines

Excellent communication skills, verbal and written, with the ability to adjust

communication style and content to suit the audience

Able to effectively manage allocated resources

Good clinical pharmacy knowledge including terminology

An appreciation of NHS agenda and government targets

Honesty, integrity and evidence of improving patient outcomes.

Evidence of being able to work well in a team

Can prioritise work and tasks when needed to suit business, PCN and patient needs Know when to ask for help from the board and senior clinical pharmacists in relation to tasks/ work assigned

Using appropriate infection control procedures, maintaining work areas in a tidy and safe way and free from hazards;

Active reporting of health and safety hazards and infection hazards immediately when recognised;

Keeping own work areas generally clean and overseeing staff in the maintenance of general standards of cleanliness consistent with the scope of their role

Undertaking periodic infection control training (minimum annually); Reporting potential risks identified
