Job summary
The post-holder will work with clinical directors and the leadership within a Primary Care Network (PCN) to create a vision to guide service improvements, identify opportunities to make tangible positive transformation, and provide the inspiration, expertise, and leadership to deliver changes in a collaborative way. They will deliver on a wide range of business, data submission, transformation and improvement initiatives across the primary care locality providing strategic transformation support and leadership.
The post-holder will be instrumental in SOLAS progressing its PCN development plan.
The post-holder will work closely with staff at practices, such as practice managers and staff at the PCN to understand current service models, identify challenges, agree priorities for improvement and then support these to be delivered.
The post-holder will develop strong relationships and engage with a wide range of stakeholders across the locality to ensure collaborative approaches to service development and delivery. In doing so they will improve integration with the broader community services as our network matures. The PCN manager will be responsible for managing agreed projects with a focus upon developing the partnerships across primary care, community services, social care, mental health, the voluntary sector and secondary care that are enabling on-going MDT development.
The PCN manager will be responsible for disseminating new information across the member practices and tasked with coordinating shared learning events. They will actively support integrated neighbourhood ways of working whilst promoting standardisation for efficiency purposes.
This role will be outward looking to help secure our resilience by actively seeking development opportunities and by coordinating proposals.
The PCN manager will be essential to recruiting and integrating new roles into our primary care teams and acting as line management for these new staff.
Working with relevant colleagues in the ICS, the post-holder will also develop relationships with local delivery partners (such as IT service providers and other NHS organisations) to support the effective delivery of transformation. The post-holder will work with the ICS to request the specialist IT and estates expertise required to deliver transformation plans in line with the ICS’s delegated responsibilities for GPIT and GP estates.
The post-holder will also work with the ICS to bring in other specialist support as required, e.g., in analytics, evaluation, organisational development etc.

Primary key responsibilities
The post-holder is to undertake the following key responsibilities in supporting the delivering of health services:
Primary Responsibilities
The core objective of the post-holder is to support the PCN and its organisations to deliver ongoing improvement to services. The role will require experience in quality/service improvement with expertise in transformational change and a good understanding of primary care systems and processes, including familiarity with general practice digital systems and how they can be used to deliver improvement. This expertise will be used to:
a. Link with other PCN transformation leads, the ICS and other system partners to collaboratively solve problems, share learning, gain buy-in for transformation and to support alignment with wider strategy and improvement approaches
b. Support organisations and staff to optimise the use of existing functionality and new technology to deliver benefits for patients and staff and to enable improvement
c. Use data to support population health management and workforce planning
d. To work as part of a multi-disciplinary team and to provide day-to-day managerial supervision to the SOLAS PCN by leading and supporting quality improvement and performance
e. To support recruitment, induction and training of new PCN staff and to ensure that all have the appropriate qualifications, registrations, training, accreditation and requisite skills to undertake the duties required of them at each practice
f. To work closely with the relevant teams and practices to support network implementation and standardisation of agreed service change, contracts, policies, pathways and quality improvement whilst acting as the point of contact for PMs with regard to delivery of a PCN project/scheme
g. To help staff within the PCN to understand and support the delivery of its strategic objectives and outcomes framework
h. To co-ordinate the delivery of enhanced services and other service submissions on behalf of the PCN
i. To provide management oversight at board/strategic meetings
j. To work alongside PMs within the PCN for the setting up and maintaining of new financial systems
k. To monitor and check the accuracy of the any submissions for enhanced services and contractual requirements on behalf of the PCN and to ensure that all claims are submitted in good time and payment made and received
l. To ensure financial risks are recognised and appropriate action taken
m. To monitor expenditure, plan and monitor income and cash flow to ensure that income is maximised and that funds are available as required and practice allocation of resources if fair and equitable
n. To present financial plans, strategic goals and any monitoring information and reports to the PCN on a quarterly basis
o. To ensure the PCN’s compliance with GDPR and FOI along with other relevant legislation and professional standards from GMC/BMA and LMC guidance
p. To manage risks and issues with supporting outcomes. These are to be reported to the PCN clinical director
q. To assist in the resolution of any complaints, resolving informal complaints locally and assisting in the investigation of and response to formal complaints as required
r. To work with the PCN clinical director and any supporting groups for strategic and operational planning processes to support the PCN’s objectives
s. To support delivery of incentive schemes, QIPP and other quality or cost effectiveness initiatives
t. To establish a business development plan
u. To create a business continuity plan
v. To develop yourself and the role through participation in training and service redesign activities.
w. To undertake all mandatory training and induction programmes
x. To contribute to and embrace the spectrum of clinical governance
y. To attend a formal appraisal with their manager at least every 12 months. Once a performance/training objective has been set, progress will be reviewed on a regular basis so that new objectives can be agreed
z. To contribute to public health campaigns (e.g. flu clinics) through advice or direct care

Information and data management
The post-holder will need to have skills in relation to understanding and using data to drive improvement and assess impacts.
These skills will be used to:
a. Take a data driven approach, looking at the evidence and information available to support an understanding of the challenges, identify possible solutions and track the impacts of changes that have been made
b. Work in partnership with the ICS to identify relevant data sets and to support standardisation in the use of measures for understanding change and improvement in general practice
c. Bring together data from a variety of sources and use information to develop insights into working practices, or support population health management, risk stratification or workforce planning, effectively tailoring content to meet the needs of the audience
d. Effectively present and communicate data to organisations and PCN staff at all levels to increase understanding of operational challenges
e. Seek and secure analytical resources as required to support the PCN in developing and implementing appropriate data and information sharing mechanisms
f. Use data to track and assess the impact of changes made to operational processes and to track trends and developments over time
Communication and engagement
With responsibility for developing and supporting the delivery of transformational projects across the PCN, the post-holder will be skilled at engaging different stakeholders, bringing people together and gaining support and buy-in to change.
Lead or participate in relevant working groups, project groups, service meetings and other forums across the PCN (and ICS) to agree changes, share learning, discuss challenges, and resolve issues.
Secondary responsibilities
In addition to the primary responsibilities, the post-holder may be requested to undertake:
Service development
With responsibility for supporting the PCN to identify and make progress with transformational change, the post-holder will be skilled at understanding and interpreting national and system- wide policy developments and initiatives to local circumstances and contexts.
These skills will be used to:
a. Maintain a good knowledge of key policies from the Department of Health and Social Care, NHS England and other relevant government departments and agencies, and support PCN staff to understand and interpret these locally.
b. Collaborate with peers in other PCNs and work with the local ICS to support the development of overall primary care transformation strategies, including digital transformation strategies, and their delivery.
c. Support the PCN and member organisations to adapt and apply specific policies and initiatives to local needs and to prioritise initiatives based on local requirements.
d. Undertake any tasks consistent with the level of the post and the scope of the role, ensuring that work is delivered in a timely and effective manner
e. Duties may vary from time to time without changing the general character of the post or the level of responsibility.
