An exciting opportunity has arisen for a Patient Tracker on Ward B6 for a 12 month secondment to cover maternity leave. This role is fundamental in supporting patient flow throughout the trust ensuring a safe discharge for our patients. The role also includes frequent contact with the whole Multi-disciplinary teams ensuring safe and timely transfers and discharges from hospitals.
We are looking for an enthusiastic, driven and motivated member of staff, to complement our existing team. Excellent communication skills and IT literacy are essential for the role.
The main focus of this role will include assisting in patient discharges, liaising with other members of the multi-disciplinary team and other Hospitals. You will make a valued contribution to the overall patient experience.
You will role model the trust values and behavioural framework and ensure discharge planning is carried out to a consistently high standard.
