Job description

This is an incredibly exciting time to join the Patient Safety Team at Northampton General Hospital.

This is a newly created role that will be fundamental to support the transition to the Patient Safety Incident Response Framework (PSRIF) and the National Patient Strategy.

We are looking for individuals who are passionate about patient safety and want to be part of a team dedicated to championing a patient safety culture.

The post holder will have a visible presence in the Trust and be key to the implementation of the Patient Safety strategy at Northampton General Hospital.

This post is responsible for supporting the effective and timely response to patient safety incidents, by working collaboratively and proactively with the Investigation Teams and Divisional staff to ensure a thorough and proportionate response in accordance with Trust, National and Commissioner’s policies and good practice.

The post holder will work autonomously as a patient safety reviewer and in conjunction with the patient safety incident investigators, subject experts, and divisional colleagues, escalating any identified risks and immediate safety actions to the Head of Patient Safety Improvement.

The post holder will be responsible for the day to day management of patient safety incident response, ensuring a systems learning approach and that patients and their carers/families, Trust staff and where appropriate, third parties are fully involved in the review process and act as a role model to develop a culture of continuous improvement.

The post holder will support the Governance team in maintaining the patient safety incident response process in the Trust ensuring it is in accordance with policy and recognised good practice.

Contribute to the development and improvement of a systems learning approach and cultural changes associated with the introduction of the Patient Safety Incident Response Framework (PSIRF) and support allocated improvement and innovation projects.

The post holder will review patient safety incidents which are reported on the Trust incident reporting system (DATIX) and other sources of insight including but not exclusively; Freedom to Speak Up, complaints and ward visits and support an immediate response and information gathering

Undertake patient safety incident reviews using a variety of different methods, including After Action Reviews, Clinical Reviews and Structured Judgement Reviews.

The post holder is responsible for ensuring that all internal and external reporting requirements and timescales are adhered to for the management of reported incidents.

The post holder will ensure that Duty of Candour is carried out in accordance with Trust and National Policy, and audit data is available to demonstrate compliance.

Apply the principles and practice of a just and learning culture across a diverse range of clinical services and recognise when specialist expert opinion is required.

The post holder will have an active and visible role in supporting Multidisciplinary Investigation Teams with the incident response process and identifying learning and improvement.

The post holder will support the Multidisciplinary Investigation team in communicating with stakeholders as required.

The post holder will work with and establish links with Divisional colleagues to ensure that sharing and learning occurs and maximise opportunity to triangulate insight to improve safety across the organisation.

Work with the patient safety specialists to maintain efficient systems and processes to ensure good governance and enable the effective and accurate collection, collation and reporting of routine data and information, producing statistics and reports when required.

Support the development of the Trust’s Patient Safety Incident Response Plan (PSIRP) and work collaboratively with a variety of staff groups and members of the public, utilising highly advanced communication and arrive at well thought out decisions.
