Job description
Responsible for leading on serious incident investigations, collating highly complex information, communicating with staff, patients and their carers/families, working closely with both individuals involved, the Care Group and the patient/service users and/or family. This may also include external organisations and stakeholders.
The post holder will use nationally accepted methodologies to produce and present high-quality reports that represent the views of patients, families and named staff. Identifying causal factors, human factors, learning and make recommendations for changes in practice as required.
Contribute to the Trust’s processes for ensuring compliance with the Duty of Candour regulations when investigating a serious incident, where required.
- Have the necessary skills and experience to conduct patient safety incident investigations (serious incidents), techniques and practice, including root cause analyses (RCAs), human factors, cognitive interviewing and applying the statutory Duty of Candour. The role will require supporting and working with patients and families, including those bereaved, to learn from incidents and concerns raised through the complaints route.
