Patient Pathway Co-ordinator (Band 4)
We have an exciting opportunity for an exceptional candidate to join our Diabetes, Endocrine & Renal Administration Team. The successful candidate will be able to demonstrate advanced communication and organisational skills, use their initiative and show problem-solving skills. Excellent attention to detail and a passion for patient care is essential.
This role will support the proactive management of patients on clinical pathways. The patient pathway requires information to be collected at various points along the patient journey. The post holder will support the collection and accurate recording of information following a patient referral which will enable the Trust to accurately identify the time from referral to treatment.
The post holder will be required to proactively monitor and validate patients on clinical pathways, supporting the Trust to ensure patient safety while waiting and operational planning of encounters and treatments, as well as supporting the Trust in maintaining performance targets.

The post holder will be the point of contact for all administrative issues relating to patients’ pathway of care. The post holder will work as part of a multi-disciplinary team and will provide expertise in the proactive management of the patient pathway from referral to discharge, ensuring the entire pathway is managed smoothly. The post holder will supervise and allocate work to the POD (point of delivery) team.The post holder will ensure high levels of patient and clinician satisfaction by being an accessible, customer focused and knowledgeable point of contact. To provide a consistent approach across the Trust, using Standard Operating Procedures (SOPs) to ensure that functions of the role are carried out correctly within given timescales.
