We are looking for someone to work as a Patient Co-Ordinator/someone who can multi task and jump into other departments if needed. We are a busy GP Surgery with a complex range of patients. We would like someone who wants to really get stuck and in and learn about all the different areas of Primary Care, who can eventually become someone the team can go to with complex queries regarding patients
You will work with all the teams, and closely with the management team and the Doctors
JOB TITLE: CARE CO-ORDINATORREPORTS TO: Doctors, Practice Nurse Team, All Managers
Job summary: To communicate closely with patients, representing their needs and supporting and signposting accordingly and appropriately. Working closely with the Clinicians in the Practice to communicate with patients as requested. Supporting the Clinical Team in all Administration tasks.
Patient Lead Responsibilities
- To manage patients with complex prescriptions needs and enquiries, ensuring a smooth system is in place that is clinically approved and ensures a safe prescribing routine for these patients.
- To identify vulnerable patients and those patients that are at continued risk of admission and where appropriate propose review into the Frailty meetings.
- To manage and organise Multi-disciplinary meetings. Liaising with multi-disciplinary teams in relation to the healthcare of patients identified for this pathway.
- Scheduling patient treatments or other procedures
- Coordinating clinical visit and follow-up visits as requested by patients and the clinicians of the practice.
- Coordination with the Practice Business Manager of reminder procedures for QOF invitations and follow ups.
- Setting up of specialist appointments including letters and records, and follow up on care and outcomes.
- To be a first point of contact for patients requiring general information relating to the practice and signposting to healthcare services.
- Provide lifestyle advice information/signposting to patients making any necessary referrals within the practice
- Participate in the administrative and professional responsibilities of the practice team
- Ensure accurate and legible notes of all consultations and treatments are recorded in the patient’s notes where the post holder is involved.
- Ensure accurate completion of all necessary documentation associated with the patients’ health where the post holder is involved.
- Attend and participate in practice meetings as required.
- Assist in formulation of Practice philosophy, strategy and policy
- Ordering and display of Health Promotion materials
- Undertake any other additional duties appropriate to the post as requested by the Partners or the Practice Manager, or Deputy Practice Manager
- Work with the home visiting team to ensure all visits are organised and any administrative tasks undertaken
- Be involved in frailty and care home LCS
Skills
- High degree of organisation and an effective time manager

- In depth knowledge medical practices, treatments, procedures and services available
- Committed to patient care and able to empathise with a wide variety of patients
General
The Patient Lead will be expected to assist clinical personnel in the care of practice patients to include treatment, preventative care, screening and patient education. The post will also include some administrative duties.
They will act as a focal point of communication between patients, and other clinical and non-clinical members of the practice.
