About us
Streatham PCN has 6 practice members, and serves a population of over 49,000 patients. We have 7 clinical pharmacists, 8 care coordinators, 4 SPLW and 2 HWBC employed under the ARRS, and are now looking to add the skills of a community paramedic, to improve the way we manage our housebound population. We are an approved educational hub, and provide opportunities for all members of the practice and PCN teams to develop their skills and roles in delivering care to patients.

Job summary
The main aim of the role is to support the triage and assessment of patients presenting urgently to practices, and to deliver a home visiting service for housebound patients, which could include delivery of routine clinical care e.g. LTC checks.
The paramedic will provide a rigorous and holistic assessment of patients presenting with unscheduled care needs as a first contact practitioner, and will treat, diagnose, discharge and refer patients safely based on assessment findings and patients own beliefs and expectations.

Job responsibilities
The Community Paramedic will work within the multidisciplinary structure (GPs, Nurses/HCAs, Clinical Pharmacists and admin team) within the allocated practice site, delivering a mixture of practice based clinics for assessing and treating patients with urgent needs and home visits to assess the urgent needs of housebound patients requiring care as well as routine visits to manage their LTC reviews/planned care needs, where these are not being provided by community services.
