The outreach worker would be joining a successful and passionate team working across two sites in Woking.
The outreach workers are invaluable to the success of overall objective to reduce permanent exclusions across the 16 secondary schools that they work with.The Trust is proud to report that since 2018 permanent exclusions across the trust have reduced by 50%. To continue this positive, strong trend, they need to recruit brilliant, passionate and inspiring people who are driven to make a difference to a child's life.

As an Outreach Worker you will:

• Work with schools to provide support children (and their families) at risk of permanent exclusion.
• To provide outreach and family support to families in most need, identifying, assessing and evaluating the outcome of the impact the intervention has achieved.
• Be aware of the services available around the Local Authority, ensuring that families in most need are supported to access these services. You will have access to the support the parents and school needs to get involved with their children’s development, health, learning and wellbeing.

Case Management

• Monitor, manage and deliver care plans in specified service area.
• Identify, devise and implement intervention strategies to prevent permanent exclusion.
• Undertake case related reports and maintain records in accordance with procedural and legislative requirements.

General Responsibilities:

• To follow agreed school policies on the curriculum and the management of behaviour
• To share in the supervision of pupils at break time, arrival and departure in accordance with published duties
• To take a share, under the direction of the Leadership Team, in the whole school approaches towards the making, reviewing and updating of school policies through formal meetings, discussion and consultation
• To make and record home visits (as required and agreed)
• To be flexible in terms of carrying out particular duties as the Headteacher may reasonably direct from time to time
