Job description
An exciting opportunity is available in our Integrated Care Service, this post oversees our Hospital Discharge team, Reablement service and Sensory Services. This is a county wide, permanent position. Within this role you will be responsible for developing and monitoring performance, maintaining effective collaborative relationships with partners and link in with our strategic Service Manager, whom you will deputise for in their absence. You will promote and embed good quality practice, ensure a strengths-based model is adopted across the service. You will have budgetary responsibility and undertake scrutiny and audit of casefiles to ensure social work practice is of high quality and standard.
