Contract Type: Ongoing contract
Pay Rate: £18.76 per hour (assignment rate)

Duties:
- Assess the functional abilities and ongoing care needs of people with physical disabilities in their homes with the aim to increase independence and improve their safety
- Set SMART goals with people that show potential for further improvement
- Provide reablement intervention under the guidance of a therapist, including teaching appropriate techniques for washing, dressing, transfers, safe mobility and meal preparation to increase independence
- Identify and order equipment and aids that can optimise function and prompt independence
- Work within a multi-disciplinary team (MDT), liaise frequently with physiotherapists, occupational therapists, district nurse and care providers providing regular feedback in the weekly MDT meeting
- Manage a caseload of clients independently

- Previous experience working as a Therapy Assistant
