Job description

About Us:

THE EXTRAORDINARY STORY OF INDEPENDENT LIVES

Adult Social Care in Westminster City Council is a world of extraordinary stories, where our passionate, caring and committed professionals do brilliant work every day.

Mrs D was frail and unwell. Her son was her main carer, but he wasn’t coping. We stepped in to help, but they were hostile to outside agencies. They’d always lived together and were terrified they’d be split up. Our social worker did a great job persuading Mrs D to get hospital treatment and a temporary stay in a care home. In an epic effort throughout COVID, she gained the son’s trust and sorted all the repairs, carers and nurses needed so that Mrs D could come home to live with her son. We also got Mr D regular respite and a personal budget awarded in recognition of him as her carer. After all, there’s nothing more important than living the life you want.

The Role:

As an Occupational Therapist Care Manager, you can make your own powerful contribution to people’s sense of independence, even when life throws challenges their way. We know that people want to get out of hospital and back to their own homes as soon as it’s safe to do so. As a key member of a multidisciplinary team of OTs and social workers, you’ll work closely with other health care professionals including reablement care providers, to make a success of early supported discharge from hospital.

In this fast paced and rewarding role, you’ll hold cases for up to six weeks. You’ll deliver skilled occupational therapy intervention to individuals in their own homes. Based on a comprehensive holistic assessment of their clinical and personal needs through an agreed, time limited goal plan, you’ll help people get back to independent living in the community in a safe and supported way. From moving and handling plans, to ordering equipment, you’ll make sure everything’s in place for a new phase of life.

As an Occupational Therapist Care Manager in a Reablement Team you will be working with clients in a goal focused, up to 6 week programme to support clients to reach their desired outcomes.

In return for the life-enhancing work you do, you can look forward to generous annual leave, excellent benefits and great scope for career development.

About You:

A HCPC registered occupational therapist with a relevant qualification or equivalent experience, you have a track record of working with adults with disabilities - ideally within their own homes. You have demonstrable skills in completing complex movement and handling assessments and care plans. Comfortable working in a multidisciplinary setting, you’ll bring to the role great communication and relationship building skills, together with a flair for problem solving. Like everyone in our team, you are passionate about enabling people to feel safe and empowered to live more independent lives.
