Come and join our friendly and supportive medical team as a Band 4 Occupational Therapy practitioner. This is a 12 month fixed term contract to cover maternity leave.
The successful candidate will be responsible for independently managing a caseload on acute wards at Musgrove Park Hospital with support from a well-established therapy team. You will need to have had experience working at Band 3 level or equivalent.
The post holder will work closely with the MDT including Doctors, nurses, physiotherapists ,discharge coordinators and Adult Social Care and will be well supported by the medical team of Occupational Therapists both with their clinical caseload and to engage with the ample opportunities for personal and service development.

Responsibilities:
- Be able to use a person-centered approach to assess the needs of patients with a variety of conditions involving physical, cognitive or emotional difficulties
- Writing appropriate goals and treatment plans. Interventions may include activities such as washing and dressing, grooming, practising transfers, equipment provision, orientation, kitchen practice or other personalised activities as well as addressing areas to minimise the impact of frailty and enable patients to manage their conditions independently.
- Supporting Band 5, 6 & 7 Occupational Therapists with interventions for complex patients
- Working with the multi-disciplinary team to support the patient's onward journey out of hospital
- Supervising and supporting other junior members of the team

The post holder will:
1. Maintain a clinical caseload with the ability to treat a range of patients i.e. those patients whose activity of daily living skills/ home situation was stable prior to admission and who have lost their independence due to an acute injury or illness.
2. Treat a designated caseload of patients as a fully autonomous practitioner with responsibility for, and legal accountability for, individual clinical decisions and actions
3. Plan and prioritise their own clinical workload, ensuring a prompt and efficient service using clinical judgement and departmental guidelines
4. The post bridges the gap between that of a band 3 occupational therapy assistant and a band 5 occupational therapist by providing intervention to a small, designated caseload with guidance and support as required.
5. Ensure all patients understand the occupational therapy process, and have given valid and informed consent for treatment. Where patients are not able to provide consent for treatment e.g. when unconscious, acutely ill, or suffering from severe dementia the post holder will work within existing legal frameworks, Trust guidelines, and professional codes of conduct
6. Use a holistic approach to treatment for a designated caseload of patients. The interventions will integrate information from a range of sources such as the patient, carers, medical/nursing records, health and social care team, community mental health teams and a basic clinical assessment
7. Take into account the cognitive, psychological, social, sensory, perceptual and motor as well as medical aspects of the patients’ care, in order to provide a patient centred approach, referring/signposting to other specialists/services as necessary
8. The patient group treated will be those who have been admitted for elective orthopaedic surgery or have been admitted due to trauma under the Orthopaedic team. There will also be general surgical/medical patients to be treated alongside Orthopaedics.
9. To identify the requirements for adaptive equipment, care and support on discharge, taking in to account individual need, eligibility and financial resources
10. Formulate a basic problem list and appropriate patient centred goals.
11. Select appropriate protocol based treatment pathways OR refer to an occupational therapist as necessary
12. Complete occupational therapy treatment protocols in order to rehabilitate the client group
13. Assess and evaluate the effects of the chosen occupational therapy treatment over time, altering the treatment modality as necessary
14. Take a full part in the patient discharge planning process, documenting occupational therapy elements of the ward discharge process.
