We are very excited to announce that the Community Treatment Team (CTT), an award-winning hospital avoidance team, are looking for innovative, passionate and charismatic individuals to join the team.
CTT is an urgent response team, due to the fast-paced nature, applicants must be keen to respond to crisis scenarios in a timely manner while ensuring a patient centred approach is always maintained.
CTT covers 3 of London’s Boroughs - Havering, Barking & Dagenham & Redbridge for adults over 18 and is based in the community and works with the BHRUT acute hospital trust to discharge patients from the Front door (ED and MRU) and Frailty units. The service aim is to assess and treat frail patients in their own homes and avoid unnecessary hospital admission. The service provides an alternative management/care plan to support the patient at home or refer them to the most appropriate pathway/intermediate care service.
Due to the crisis nature of the role the candidate will be supported to extend their skills in order recognise a deteriorating patient. Close team working and effective communication is paramount in order to manage complex patients at home.
The candidate will be supported to undertake training in order to competently assess holistically and order equipment that is necessary to keep a patient safe at home allowing them to complete activities of daily living.
Work closely with the Occupational Therapists / Physiotherapists in the team to assess, implement and evaluate crisis interventions
To support patients with complex needs to remain in their own home and avoid unnecessary admissions.
The role will require the successful candidate to work as part of the team in the community and the Emergency Department (ED). The candidate will assess patients determining their care needs in order to manage them through their crisis.

This post will be subject to a probationary period. Internal applicants are exempt from the probationary period (unless you are an internal applicant currently part way through a probationary period or currently a bank member of staff).
Starting with NELFT
NELFT place a great deal of importance on new starters being properly welcomed and inducted into the Trust. All new starters will join the Trust on the first Monday of each month and will undertake a comprehensive induction of up to two weeks which will include mandatory training, systems training and the allocation of equipment. As part of the process, new starters will have the opportunity to also meet the executive team, senior managers and attend a number of drop-in sessions focusing on engagement, health and wellbeing and key processes. The induction will be held at our head office in Rainham, Essex.
High-Cost Area Supplement - London
This post also attracts payment for High-Cost Area Supplement of 15% of the basic salary (minimum payment of £4,313 and a maximum payment of £5,436).
In all areas the candidate will need to work using current evidence based therapy techniques, providing thorough and rapid assessments, facilitating a quick and effective discharge home or avoiding ED attendance. The candidate will also need to engage with other MDT members internally and externally. CTT consists of Occupational Therapists, Physiotherapists, Health Care Assistants, Nurses, Doctors, Pharmacists and Social Workers.
The band 4 will be expected to supervise and manage band 3 staff and be supported to develop and deputise for team members of staff in their absence.
