Job description
Work within the Community Neuro Rehabilitation Service under the direction and guidance of the Occupational Therapists in the process of client assessment, treatment, and ongoing rehabilitation, working towards client centered goals as part of the multidisciplinary team.
The service will support independence by providing rehabilitation after an acute illness, injury or change in life circumstances. It aims to restore, maximise or prevent deterioration in physical, psychological and social functioning through episodes of rehabilitation.
KEY RESPONSIBILITIES
- Provide high quality assessment and treatment to adults referred to the Community Neuro Rehabilitation Service.
- Deliver individualised treatment programmes to adult/elderly clients referred to the Community Neuro Rehabilitation Service as directed by lead therapists.
- Work closely with the multi-agency team to meet the needs of the clients/patients and enable them to receive a comprehensive service.
- Complete assessments and retain case holding status for clients (under supervision) where the client requires progression of clearly stated rehabilitation goals handed over by a referring therapist where recovery is predictable, and protocol based.
