Occupational Therapy Assistant – Reablement
We are looking for talented, motivated and ambitious people to join our growing team. Our practitioners support people in The East Riding of Yorkshire to maximise their independence through focused and goal orientated intervention and by working with individuals to increase their ability to look after themselves following illness, injury or with long term conditions and to rebuild the skills and confidence to enable them live as independently as possible. We are working closely with colleagues and partners in health to ensure that we have an integrated and collaborative approach to our service.
As an occupational therapy assistant you will undertake strength-based assessments in the community where the individual sets their own goals and agrees a plan to meet these with support, help and guidance. You will have your own caseload and work under the supervision and support of a registered occupational therapist. You will work as part of our community support service which provides reablement, recovery and rehabilitation for people leaving hospital and supporting people to maintain their independence in the community. We are partnering with Hull University Hospitals, Hull City Council and the Community Health Care Partnership to deliver a new and innovative discharge to assess model. As part of the team you will be central to the success of this new venture.

You will need to have a full driving license and the ability to travel throughout the East Riding and so access to a car is essential.
