- All roles will attract a generous shift allowance and a Local Weighting Allowance of £1,002 per annum (pro rata).

- Hours of work for both roles: Full time 37 Hours, on a shift pattern with some evening and weekend working

You will be key support for service users in achieving increased independence, working to promote their wellbeing and daily living skills by teaching, assisting in personal care and motivating people in re-learning activities as part of the outcome based Reablement plans designed by Occupational Therapists. What you’ll be doing:
- Using Occupational Therapy expertise and supporting other therapy staff in assessing, implementing, evaluating and reviewing plans for service users and their carers, enabling them to achieve maximum independence in their own homes.
- Manage a clinical caseload of clients, who have physical or mental health problems, learning difficulties, dependencies or who are elderly at a specialist level, assessing and treating clients and maintaining clinical records autonomously.
- To work collaboratively with health and social care community and acute colleagues, including Local Access Point and Discharge to Assess teams.
For the Senior Practitioner role, you will additionally:
- Take a lead role in assisting the OT Service Manager to run the therapy element of the Reablement and Independence Service.
- Support with the transfer of cases between our integrated pathways.
- Line manage and provide clinical supervision to therapy staff within the service.
- Work closely with the Local Access Point.
- Provide line management for therapy staff and chair MDT meetings and case reviews, working autonomously
What you’ll have:
- Experience of working with people of diverse cultural and religious communities
- Excellent time management skills, including the ability to work to deadlines and work well under pressure
- An ability to work with staff and managers at all levels across the community, engaging with health, housing, partner agencies, and voluntary sector
- Knowledge of relevant legislation, national guidelines and best-practice for a range of conditions
- Requirement to work on a shift basis including evenings and weekends
- MSC/ Bsc/Diploma in Occupational therapy
- State Registration (HCPC & College of OT)
- Full Driving Licence with access to a car on a daily basis
