Based in Ipswich at Mariner House. Our forward-thinking community mental health teams are seeking a band 5 Community Mental Health practitioner to join their team. Working with young people and their families between the age of 18 - 25 years.
We are currently improving on how we work, to ensure that our team can provide an excellent service to our local communities; we have recently invested in clinical lead posts to support our current multi-disciplinary teams to ensure we have clinical nurse specialists and are able to support those recently qualified.
As a mental health practitioner , you will be working with service users with mental health conditions, that require ongoing care and treatment from secondary mental health services. We look to seek new ideas and ways forward and would welcome your experience and views.
It is the ideal time to join our collaborative team as we transform and develop the way we work -although this is a full-time role, we can be flexible and can offer some hybrid working and/or flexible working patterns if needed.

You’ll play a key role in our service users recovery plan, you’ll complete a recovery care plan collaboratively with them and review this regularly with the aim of the service user being discharged from the service as part of their recovery journey. You will work with the service user, carers, external agencies and other services to ensure the service received is streamlined and cohesive.
You’ll be working alongside a multi-disciplinary team and part of your role will be to deliver assessments and provide treatment, which we will teach and develop with you. You’ll regularly be working with external agencies as part of a service users care, so being able to work well in a team and having excellent communication skills is vital. You can expect a thorough induction and training period, as well as opportunities for ongoing development.
