Job description

The post holder will participate in the shared caseload of one of the Trust’s multidisciplinary Community Mental Health Teams in Later Life Services in South Manchester. The post holder will undertake occupational therapy needs assessments and contribute to the risk assessment of service users referred to the Team, who may have either time-limited mental health disorders or severe and enduring conditions. They will act as a care co-ordinator or case manager for service users who require Occupational Therapy.

They will develop and negotiate care plans with service users, families/carers and other professionals and agencies, ensuring access to the appropriate interventions and opportunities, with the aim of promoting social inclusion and recovery.

The post holder will carry out evidence based occupational assessments and write up reports of a high standard, which highlight how biological sociological and psychological factors interact to create obstacles to the satisfactory participation of an individual service user in valued social roles.

The post holder will also undertake Duty Triage and Duty Assessor roles.

  • Accept occupational therapy (OT) referrals and provide OT to clients of the CMHT.
  • Be aware of and practice within the relevant legislative clinical guidelines issued by local and national statutory bodies and be legally responsible and accountable for all aspects of work.
  • Select and undertake appropriate Occupational Therapy assessments appropriate to the needs and presentation of the client and which explore the biological, psychological and social factors known to impact on performance.
  • Apply theoretical knowledge and understanding of psychology, sociology physical science and of the effects of disability on function to assess and develop a case formulation describing an individual’s strengths and obstacles to occupational performance.
  • Contribute to the diagnosis and team formulation of clients.
  • Identify client goals collaboratively, and plan and implement evidence based interventions to use strengths and overcome obstacles to goal attainment aimed at recovering socially valued roles and social inclusion. Carry out interventions in environments most suitable to the therapy and the aim of social inclusion
