Ealing Intermediate Care Service is expanding and as a result a new Senior Advance Nurse Practitioner role is created to clinically lead the community urgent response team also known as Rapid Response.
Rapid Response is part of Intermediate Care within Ealing Community Partners (ECP). The team includes; doctors, nurses, therapists, rehab assistants / health care support workers. They are based at Clayponds hospital and delivers high quality urgent care to patients in the locality to avoid unnecessary hospital admission and A&E attendances.
This role will provide clinical / professional leadership role to and some management support for the whole team. The post holder will work closely with the Team Manager, Service Manager, and other service leads within ECP and across Northwest London Integrated Care Service (NWL ICS) in the delivery of excellent care to patients at the right time in the right place, as part of the NHS Long term plan.
The post holder will be an integral part of the Senior Management team within Intermediate Care Service. You will be responsible in providing excellent care to patients, role model a compassionate and inclusive leadership for the team and promote patient’s safety and positive experience through evidence-based practice.
You will be the first point of contact for an expert clinical advice to your team and seek advice from the medical team when required. You will work closely with the Team Manager to ensure best practice is adhered to and performance targets are met. You will lead clinical audits and facilitate any required quality improvements within the service.
You are responsible in supporting your team to ensure everyone is performing at a high standards and in keeping with their individual job description, professional ethics and code of conduct and Trust Policies. Embed best practice and foster a supportive work environment where people develops and maximise their potentials.
