Job description

Please see attached job description for detailed duties. Relevant experience in a health setting with at least two years experience in a relevant setting. Must be a Registered General Nurse or a qualified registered AHP therapist. Able to work alone and as part of a team.

Dealing with difficult conversations when required. Assessing patients for further care, working with stakeholders, challenging and reviewing information with stakeholders. Supporting and facilitating system flow. Able to commute across county when required.

To be able to work flexibly to accommodate the service. Ability to problem solve.
