This is an exciting opportunity for a Band 3 discharge coordinator to join a motivated team that are dedicated to the development of skills and the delivery of high quality safe care.
We are looking for an individual who is self-motivated, content with working in a busy environment and who has good time management skills.
You must have excellent written and verbal communication skills, with the ability to work autonomously and as part of a team.
It is essential that you act professionally at all times, have excellent interpersonal skills and the ability to prioritise work load and recognising simple or complex needs, coordinating the discharge or transfer process together with the multi-disciplinary team.
The successful candidate must have excellent communication skills. Experience in the use of hospital I.T. systems would be desirable though not essential as full training and support would be given.
Main duties of the job
Ensure that Nervecentre is kept up to date in relation to all and any relevant information relating to the patients discharge including medically safe status and transfer information.
Under the supervision of the nurse in charge, facilitate ward admissions by effective discharge planning. (Supporting effective discharge planning to provide, smooth timely discharge)
Maintain a regular presence on the ward, attending daily Board rounds and any multi-professional case conferences. Identify patients who could be discharged, enabling these patients to be reviewed first.
Take responsibility for ensuring the discharge plan information is reviewed, assessed and updated according to patient condition changes and inform the senior medical and nursing staff if patients do not have a documented management plan within 24 hours of arrival to the base ward

Detailed job description and main responsibilities
To work with clerical and clinical staff to maximise the efficient and effective utilisation of the Trust’s beds.
To have responsibility for co-ordination of the ward discharge processes.
To ensure patients, their carers and members of the MDT are fully involved at all stages of the patient's admission / discharge process.
To utilise IT/information within the trust to facilitate better discharge on the ward and enable the ward to alter processes to improve discharge
Manage the coordination of the more complex discharges from the ward ensuring ward NIC is kept up to date with any relevant changes or issues relating to that discharge
The ability to input and provide accurate data and reports would also be desirable. If you feel you have the qualities required to fulfil this role and you are dedicated to high quality, patient centered care we would love to hear from you.

Salary: The quoted salary will be on a pro rata basis for part time workers.
