Job description
2.1 To manage a complex caseload of service users with an acquired brain injury and to co-ordinate care in the context of relevant health and social care legislation and policy frameworks including but not limited to: Care Act 2014Mental Health Act 1983/2007Mental Capacity Act 2005Interagency Adult Safeguarding policies and proceduresCHC Guidance2.2 To act as a link for the Trust across community and acute settings to ensure collaborative working between all sectors to ensure care coordination and discharge planning is achieved in line with Key Performance Indicators and relevant health and social care legislation.2.3 To undertake assessment of need from point of referral to discharge. To compile detailed assessments, reports and care plans in partnership with service users and their carers, which can also be used by Brokers and service providers to commission services to meet identified health and social care needs in line with eligibility criteria of relevant health and social care legislation and partnerships agencies.2.4 To anticipate, assess and manage risk, including complex cases and support others to develop risk management plans2.5 To work in partnership with service users and carers to promote asset strength based care to meet health and social care needs which arise out of the acquired brain injury, and to navigate and signpost to appropriate service provision.2.6 To support service users by focusing on them achieving positive outcomes to enhance their wellbeing, promoting prevention and increasing independence and championing the role of social care/prescribing in recovery pathways and delivery of therapeutic interventions on an individual and group basis.2.7 To work collaboratively with other members of the multi-disciplinary team, providing social work/care related guidance, knowledge and joint working as required.17/4/192.8 To engage with service users and families to ensure they are aware of rights in line with relevant health and social care legislation and take a lead role associated with carers.2.9 To promote advocacy and empower service users and carers to exercise their rights.2.10 To be a champion for equality and diversity and promote anti discriminatory practice associated with the protective characteristics as defined by the Equality Act 2010.2.11 To ensure carers assessments are offered and completed as required and to take a lead role to support and promote carer involvement.2.12 To take a key role in the MDT as Safeguarding Ambassador to assure the Safeguarding of Children and Adults are adequately considered within the Multi-disciplinary team. To initiate appropriate statutory action to protect service users at risk, carrying out any related preventative and therapeutic activity appropriate to the Social Work role.2.14 To act as a Best interest Assessor, or be prepared to undertake the BIA training, and act as lead with the MDT for Mental Capacity.2.13 Maintain all records to a high standard in accordance with relevant legislation, policy and procedures.2.15 Supervision and leadership of junior staff, including acting as Practice Educator for student Social Workers.2.16 Manage work to ensure personal safety and security of self and others and contribute to development of safe working environment.2.17 Maintain professional practice by keeping up to date on social work research and legislation and practice development, and if appropriate undertake or contribute to research.2.18 Develop knowledge and practice and provide evidence of continued professional development by means of a professional portfolio.2.19 Participate in and contribute to service governance and development.3.1 All information relating to patients and/or staff obtained during employment with the Trust is to be treated as confidential and as such employees should not disclose it without appropriate prior authorisation. Employees must not intentionally or recklessly interfere with, or misuse anything that is provided in the interest of the health, safety and welfare of staff, patients and the general public.17/4/198 INFECTION PREVENTION AND CONTROL8.1 Infection Prevention and Control is everybodys business, and all employees have a duty to act in accordance with the standards and procedures as set out in the Mersey Care NHS Trust, Infection Prevention and Control Policy at all times.9 STANDARDS OF BUSINESS CONDUCT9.1 The post holder will be required to comply with the Trusts Standing Orders and Standing Financial Instructions and, at all times, deal honestly with the Trust, with colleagues and all those who have dealing with the Trust including patients, relatives and suppliers.10 SAFEGUARDING CHILDREN AND VULNERABLE ADULTS10.1 Safeguarding Children and Vulnerable Adults is the business of everyone employed by Mersey Care NHS Trust. All staff and their Managers/Supervisors must be responsible and accountable for their safeguarding practice and proactive in identifying and reporting safeguarding concerns.10.2 It is the responsibility of each individual staff member to ensure that they attend mandatory safeguarding children and vulnerable adult training at the appropriate level.11 OTHER11.1 You may be required to work at other locations within the Trust as determined by the duties of your post.11.2 The Trust operates a No Smoking Policy.11.3 You may be required to undertake any other duties at the request of the line manager which are commensurate with the role, including project work, internal job rotation and absence cover.11.4 This job description is an outline and account of the main duties.
Any changes will be discussed with the post holder in advance but will also be reviewed regularly to take into account changes and developments in service requirements.
