Job description

Our MDTAs work within the community team to support practitioners (registered nurse, Physiotherapist, Occupational therapist, social worker, Clinical Nurse Specialist), to provide person-centered support, care and treatment to patients, their families and carers across the community who have palliative and end of life care needs.

The ideal candidate for this role will be able to demonstrate experience as a health care assistant in a clinical environment and be confident in contributing to the multi- disciplinary plan of care for patients, families, and carers for whom the post holder has delegated responsibility.
