Hospital at Home is an exciting and innovative service developed by the Thames Valley CAMHS Provider Collaborative, which covers Oxfordshire, Buckinghamshire, Berkshire, Gloucestershire, Swindon, Wiltshire and BaNES.
The Hospital at Home team supports young people with eating disorders who meet the threshold for inpatient care. In some cases, admission will be prevented and in others, young people will be stepped down to this model following a short inpatient admission. Hospital at Home maximizes the use of new technology, provides care close to home, reduces admission, length of stay, waiting times and provides earlier discharge from inpatient care.
As a registered nurse within the Hospital at Home team you will join a supportive , skilled and compassionate team with strong values and a commitment to improving outcomes for young people with eating disorders.
**The majority of patient care is delivered remotely using Microsoft Teams. Office bases are available in Oxford & Swindon **
The role of the Registered Mental health nurses will be:
- To ensure the delivery of a responsive high quality, safe and patient focused service.
- To work effectively and in partnership with key members of the Multidisciplinary Team to promote and facilitate effective patient care and discharge.
- To influence the development and implementation of policies, protocols and guidelines relevant to the service.
- To develop and sustain close and effective working relationships with partner services.
- To support registered and unregistered nursing staff in their professional development and in the promotion of best practice.
Hospital at Home has a strong team approach to service delivery, this includes protected time for all staff to attend daily handovers, weekly ward rounds, service development slots and regular team and individual supervision.

- To supervise and support the healthcare support workers and nursing associates in their day-to-day work and promote the concept of interdisciplinary team working.

- Deliver and evaluate specialized individual programmes of care
- Advising, where appropriate, on changes to treatment and management
- To assess, plan and evaluate individual treatment and risk management programmes of care in partnership with patients, carers and other health and social care professionals in a response to planned and urgent
- To communicate effectively with clients /carers and colleagues to agree treatment
- To ensure understanding of conditions and treatment
- To deliver a range of clinical skills which may include, for example, physical examination, ordering, performing and interpreting common screening and diagnostic tests.
- To utilise clinical expertise in identifying patients’ needs specifically around the early signs of deterioration to ensure acute changes in condition are responded to appropriately
- To complete accurate documentation of all relevant interactions in line with professional body/ guidance
- Ability to advocate for patients and others
- To act as an information and educational resource to all members of the Multidisciplinary
- To facilitate the Multi Disciplinary Team meeting and ensure key issues are shared across the MDT
- Responsibility for the monitoring of efficient and effective use of
- Ensure patient safety and Safeguarding policies and procedures are adhered to.
- Attend the multidisciplinary team meeting.
- Care planning, risk assessment and management, discharge planning.
- Writing of reports as required.
- Shared responsibility for management of resources, including ordering stock, allocating supplies to others.
- To work with relevant professionals to develop evidence-based nursing practice that is relevant to the service.
- To lead and develop educational opportunities to promote the effectiveness of individual patient care.
- To provide high quality supervision to junior staff members on a regular basis
- To be responsible together with the Modern Matron to ensure that all nursing staff receive an annual performance review and personal development plan.
- To actively participate in an annual performance review and development of a personal development plan.
- Contribute to the development of robust outcome measure collection and the demonstration of safe, high quality care.
