This position is responsible for ethical assessment, diagnosis and treatment planning for children and youth 0-20 years old that are medi-cal beneficiaries or are uninsured, depending on program placement. CAPC is the specialty contractor for children 0-5 years old; this position is responsible for providing services to this population that demonstrates expertise of child development and trauma-informed sensitivity to ensure that families This position is also responsible for coordinating and providing medical necessity evaluation, case management, intensive care coordination, and intensive home-based mental health services as appropriate. Services may be school & community-based. Bilingual preferred, not required. MINIMUM REQUIREMENTS / QUALIFICATIONS: Education: Graduation from an accredited college or university with a Master’s Degree in social work, psychology, counseling or related field. Must be registered with the California Board of Behavioral Sciences and in possession of ACSW, AMFT, LCSW or LMFT; and maintain active registration while employed. LPCC/APCC candidates may be considered, based on supervision requirements and coursework completed as outlined by BBS and scope of work on an assigned contract. All candidates must pass the Law & Ethics exam as outlined by most recent BBS regulations. Experience: Two years of relevant experience working with at-risk children and families in therapy and case management. Experience must include no less than at least 1 year of internship or volunteer experience in mental health capacity. Knowledge of: The child welfare system, childhood development, child abuse issues, DSM 5 and BBS regulations as it relates to licensure. Description of Duties: 1. Represent the agency in a consistently professional manner, always modeling a commitment to the agency’s mission and values. 2. Maintain confidentiality of records relating to clients’ treatment, consistent with state and federal laws. 3. Provide assessment, diagnosis and treatment of clients ages 0-20 years old, depending on program placement, congruent with DSM 5 implementation. 4. Complete psycho-social assessment with client and parent/guardian to determine appropriate course of treatment and severity of presenting concerns. 5. Coordinate and facilitate Child, Family Team (CFT) meetings in accordance with medi-cal guidelines and contractual timelines. 6. Provide client-focused, strength-based services at all times. 7. Utilize a variety of treatment modalities, including those specifically outlined in the Outpatient Scope of Work, to serve clients and enhance their level of functioning. 8. Assist with behavior modification plans and early intervention mental health services for the 0-5 population, as assigned. 9. Consult with psychologist/psychiatrist or other specialists concerning treatment plans and amend plans as required. 10. Provide documentation that is consistent with medi-cal, county and agency standards. Meet all required service provisions, as identified by contract, agency and medi-cal expectations. 11. Maintain regular contacts with other agencies/professionals regarding client progress in the therapeutic treatment program through phone calls, letters, and/or monthly reporting. 12. Maintain regular contact with clients, referral sources, etc. and assist in identifying challenges and finding relevant resources to support client needs. 13. Complete tasks and assignments in a manner consistent with contractual obligations. 14. Participate in Clinical Supervision as assigned, consistent with BBS regulations. Demonstrate commitment to collection of hours toward licensure; submit hourly logs to Supervisor as indicated in department policy. 15. Maintain and prepare all reports, spreadsheets, etc. as required by funding sources for Program Manager in a timely and accurate fashion. 16. Maintain caseload of up to 25 or more clients at any time. Actively manage caseload to ensure clients do not remain stagnant for long & ensure that department policies and procedures are upheld at all times. 17. Maintain billable rates of no less than 55% productivity. Demonstrate ability to appropriately use correct billing codes and note writing strategies that do not result in disallowances during audits, etc. 18. Demonstrate proficiency in utilizing the San Joaquin County assigned electronic medical health records database, Clinicians Gateway, independently. 19. Promote and maintain a sense of teamwork between Clinical Department and other CAPC staff, volunteers, Board members and other individuals relevant to the successful operation of each program. 20. Assist the Clinical Director and/or Program Manager as requested. 21. Network with other community agencies and participate, when requested, in community and health fairs and other events designed to promote CAPC programs. 22. Other duties, both program-specific and CAPC related, as assigned.
