The purpose of this post is to work as a member of the multi-disciplinary team within a prison healthcare setting, working with service users with mental health problems, focusing on the direct needs of service users, working across boundaries of care, organisations and roles coordinated through the Care Programme Approach.

The role is to provideSupport, giveTime to an allocated group of service users and thus promoteRecovery to facilitate discharge from the recovery & rehabilitation service into the community. To assist qualified staff members from the recovery & rehabilitation service, and care coordinators in the community in assessing, planning, implementing and evaluating individual care plans.

To achieve the objectives of this role, the post-holder must have the individual service user needs at the fore at all times and use the agreed set of values and skills to underpin their day to day work.

Clinical:
  • To ensure that service users engage effectively with the agreed Care Plan and access appropriate services provided on a regular and consistent basis
  • Provide information on health promotion
  • Help to identify early signs of relapse by monitoring the service user’s progress, level of functioning and mental state and alert the appropriate staff involved in the service user’s care.
  • To assist in the delivery of the therapy programme within the service by co-facilitating groups
  • To liaise with and refer service users to other healthcare services to ensure continuation of appropriate treatment
  • To be actively involved in regular multi-disciplinary risk assessments along with daily informal risk assessment when carrying STR Worker interventions
  • Report regularly to qualified staff
  • To maintain adequate records as required by existing procedures, entering appropriate details on the service users case notes as necessary
  • Participate in the review of the service provided to clients overall by STR worker and advise any changes to the supervisor
  • To provide an out-reach service, in conjunction with the service-user’s care coordinator, for clients discharged from our 24-hour healthcare facility for a defined period after discharge
  • To undertake such other duties as may be determined from time to time within the general scope of the post
Professional:
  • To attending and actively participating in training sessions and team/CPA review meetings.
  • Receive and utilise clinical supervision to develop and maintain safe practice
  • To dress and behave in a professional manner appropriate to the role
  • To be able to function effectively as a member of a multi-disciplinary team
  • Liaise effectively across services.
  • To fully participate in the Trust’s performance review and personal development planning process on an annual basis.
  • To achieve and demonstrate agreed standards of personal and professional development within agreed timescales.

  • Ensure service users are treated with dignity and respect
  • To contribute positively to the effectiveness and efficiency of the teams in which he/she works.

  • To be aware of your responsibilities in line with the 1983 Mental Health Act and other relevant legislation
Educational:
  • Undertake training as necessary in line with the development of the post and as agreed with line manager as part of the personal development planning process.
  • To participate in the service line learning & development meetings and training
