Job description

An exciting opportunity has arisen for Senior Health Care Assistants to join our new hospital virtual ward team service, to play a key part in the roll out of our digital virtual ward expansion. Virtual wards are a National initiative that allow patients to be discharged safely from hospital and then be closely observed using remote monitoring technology in the community.

The main aspect of the role will be to work with patients just before their discharge to set them up with and teach them how to use the virtual ward technology so patients are able to use the digital system and safely enter their information and details once they are at home.

These roles will form part of the hospital virtual ward team to onboard and admit patients to the virtual wards. This will involve working with the multi-disciplinary team predominantly at the Leicester Royal Infirmary hospital site to help identify suitable patients for the virtual ward prior to their discharge from hospital.

Applicants need to be motivated and enthusiastic with excellent communication skills and need to demonstrate they are hardworking and conscientious as well as friendly and approachable. An excellent understanding of using technology and IT is essential this is a vital aspect of this role.

The successful applicants will work within the multi-disciplinary virtual ward teams and be supported by the team leads and clinicians for the relevant services. The posts are permanent and may involve travel across Leicestershire.

  • Provide clinical and non-clinical support to the multi-professional team under the direction of an appropriate registered health care professional.
  • Deliver direct and indirect care as delegated under the direction and supervision of an appropriate registered health care professional.
  • Support the delivery of effective health care to patients/service users by undertaking duties as identified by the multi- professional team.
  • Undertake specified and unsupervised aspects of care and therapeutic intervention as delegated by the registered health care professional within an agreed protocol, sphere of responsibility and after completing appropriate competency- based training agreed by the Trust.

KEY RESULT AREAS

1. DELIVERY OF EFFECTIVE HEALTH CARE WITHIN THE ORGANISATION

1.1 Undertake specified direct and indirect care and clinical and therapeutic interventions as delegated by the Registered Nurse/health care professional.
1.2 Carry out specified delegated clinical care and support consistent with the patients’/ users’ plan of care.
1.3 Deliver unsupervised specified care and interventions as prescribed by an appropriate registered health care professional in accordance with agreed Trust and CMG policies, procedures and protocols.
1.4 Assist the registered professional in developing care plans; assessing and documenting care and providing verbal and written reports in an effective and safe manner.
1.5 Make observations of care with regard to supporting the patients’/users’ general health and wellbeing.
1.6 Maintain effective communication with the Registered Nurse/health care professional regarding issues relating to assigned duties, promptly reporting areas of concern.
1.7 Assist and support care delivery by only undertaking delegated work that is within own level skill and competence.
1.8 Carry out duties consistent with individual patients’/users’ wishes, maintaining privacy, dignity and recognising cultural diversity and needs.
1.9 Contribute to the overall effectiveness of the care delivered by the health care team and which benefits the patient/user experience.

2. MANAGEMENT AND USE OF INFORMATION, RESOURCES, CLERICAL AND ADMINISTRATIVE ACTIVITIES

2.1 Use health-related information only for the purposes for which it is intended and in accordance with the Data Protection Act.
2.2 Ensure confidentiality is maintained at all times and information relating to patients/ users and staff is used only in connection with authorised duties.
2.3 Carry out designated clerical duties as delegated by the Ward/Unit Manager, Registered Nurse/health care professional.
2.4 Assist and contribute to the efficient use of stock, sundries and resources within own work activities.

3. MAINTAINING A SAFE ENVIRONMENT AND QUALITY

3.1 Take responsibility for maintaining own and others’ Health, Safety and Security.
3.2 Ensure own actions assist with maintaining best practice and Standards for Better Health.
3.3 Promote and practice the highest standards of the prevention and control of infection at all times.
3.4 Assist with the maintenance of a clean and safe environment adhering to prevention and control of infection policies, procedures and best practice.
3.5 Carry out duties in accordance with the Health and Safety at work Act 1971.
3.6 Move and handle patients and goods in ways that promote the health of the patient and care team and are consistent with legislation.
3.7 Assist and maintain safe and hazard-free area of work.
3.8 Act in a professional manner and adhere to the UHL uniform policy.

4. CLINICAL GOVERNANCE, REDUCTION OF RISK, AUDIT AND RESEARCH

4.1 Undertake only those duties and delegated tasks and treatments within own limitations, level of competence, skill and training.
4.2 Deliver direct and indirect care within Directorate, Trust, local and nationally agreed protocols and policies.
4.3 Assist with the collection of simple data for audit/research and other purposes as delegated by the health care team.
4.4 Contribute to the management of risk through risk assessment processes to ensure that risks are reduced at all times to the lowest minimal levels.
4.5 Adheres to Trust policies, procedures and guidelines and promotes the compliance of others.

5. ESTABLISHING EFFECTIVE RELATIONSHIPS AND COMMUNICATION

5.1 Maintain healthy working relationships with the multi-professional team, patient and users.
5.2 Contribute to the effectiveness of inter-agency working as appropriate.
5.3 Support patients and users in maintaining their identity and relationships.
5.4 Ensure that lines of reporting to the Registered Nurse/health care professional are timely, robust and efficient.

6. RESPONSIBILITIES FOR CONTINUING EDUCATION AND PERSONAL DEVELOPMENT

6.1 Undertake the Trust Corporate and Directorate specific Induction and competency programmes appropriate to role.
6.2 Take responsibility for undertaking own mandatory training and updates as required.
6.3 Undertake and complete specified training and/or equivalent competency programmes relevant to CMG needs and own role.
6.4 Complete specialist training required that allows health care to be delivered to the user through new ways of working.

Take responsibility in collaboration with line manager within the appraisal / Personal Development Planning (PDP) process for identifying own learning and development needs within the knowledge and skills framework (KSF).

6.6. Demonstrate skills and activities to others in similar roles and make a contribution as a team member to the training of Health Care Assistants, support workers and supervising learners.

6.7 Undertake work-based assessor awards and act as an assessor of appropriate learners/ cadets and others as appropriate to own role.

6.8 Participate in Clinical Supervision and act as a supervisor/mentor for those in similar role.

1. DIRECTORATE /ROLE SPECIFIC

Following a period of competency based training and under the direction of a registered health care professional the post holder will:

7.1 Work with the Multidisciplinary team to identify suitable patients for the ‘virtual ward’

7.2 Admit patients onto the relevant ‘virtual ward’

7.3 Prior to discharge onto the ‘virtual ward’ to set patients up with and teach them how to use the virtual ward technology so the patients are able to use the digital system and safely enter their information and details once they are at home.

7.4 Assist with essential nursing care.

7.5 Perform vital sign observations, accurately document and report back to a registered nurse / health professional.

7.6 Perform Electrocardiograph (ECG) tracings - ensuring it is checked by doctor, advanced nurse specialist.

7.7 Insert cannulae and flush with normal saline after checking the normal saline with a registered practitioner.

7.8 Obtain blood samples, label appropriately and send to labs.

7.9 Process in department blood results (Near Patient Testing) ensuring a doctor checks the result.

7.10 Escort / transfer patients to other areas of the hospital as appropriate

7.11 Perform basic wound care / dressings

7.12 Prepare patients in the final stages of discharge for example the booking of hospital transport, prompting the MDT to prepare discharge medicines (TTO’s)
