Job description
This position is a hybrid remote position. The successful candidate must be able to come to the office at least once monthly for training and support.
Scope of the Job
Under the supervision of the Value Based Care Director, coordinates hospital discharge, transitional care management, and maintenance care with primary care. Performs clerical patient care services for care coordination interdisciplinary collaborations.
Essential Functions
- Schedule and confirm health maintenance, hospital discharge, and transitional care appointments. Perform follow-up and rescheduling calls for missed appointments. Utilize external notification system to arrange follow-up care with primary care physicians. Provide overflow support contacting patients not seen in the last 3 years.
- Monitor delivery of care by documenting appointment outcomes and maintaining communication with manager.
- Evaluate and report outcomes with interdisciplinary team to design corrective action.
- Contribute to interdisciplinary collaborations with Managed Care team.
- Support patient experience initiatives designed to improve provider star rating.
- Update and maintain patient data to assist in the assigning of patient risk scores. Activate care team creations for patient with high risk scores.
- Assist in completing social determinants of health questionnaire and My Chart activations.

Required to make independent judgments without supervision. Must be able to generalize, evaluation, or make decisions based on sensory or judgmental criteria. Can adapt to perform a variety of duties, often multi-tasking without loss of efficiency or composure. Requires the ability to work with people beyond giving and receiving instructions.
Skills
- Ability to organize and prioritize assignments to work independently as well as a collaborative team member.
- Basic computer literacy and arithmetic skills.
- Knowledge of ambulatory health care or related health care field.
- Ability to provide dynamic customer service using various techniques including positive language, calming presence, attentiveness, clear communication, and patience.
- Possesses critical thinking, prioritization, and organizational skills.
- Attention to detail and quality of work.
- Ability to communicate effectively and courteously, both orally and in writing.
Experience
5 years of Medical Assistant experience required. 2 years of clerical/customer service experience - ideally in a health care related industry required.
Education & Certifications
High school diploma or GED equivalent required. Graduation from approved Medical Assistant program. ICD-9 and CPT coding preferred. Completion of a standard terminology course would be helpful, but not required. Nationally Certified as a Register Medical Assistant within 6 months. Current BLS Certification.
