Job description

Key Responsibilities Provide an effective medicine supply service to wards/departments and patients Working as part of a multidisciplinary team to ensure that patients have access to the correct medicines at the right time and that they are involved in decision making Identify and order medicines as part of the ward stock top-up service Be responsible for the receipt and issue of goods in the pharmacy and checking of delivery notes and invoices as specified Accurately dispense discharge prescriptions and other medication orders, including extemporaneous prescriptions, unlicensed drugs, controlled drugs, in-patient and outpatient prescriptions in accordance with safe systems of work practice Ensure that patients own medicines are brought into hospital through direct discussion with patient/relatives/carers Providing information to patients on their medicines supplies within remit Ensuring patients have adequate supply of medicines and reordering as necessary Liaising with discharge facilitators and multidisciplinary teams to identify patients requiring medicines for discharge Preparing medicines for discharge and dispensing items where applicable Acting as a point of contact for basic pharmacy related queries from other staff including nurses and multidisciplinary teams Interpret pharmacy portal reports and prepare daily handover for pharmacy ward teams Review antibiotic reports and order stocks as required through the pharmacy portal system Review Controlled Drugs report and identify order requirements and liaise with nursing staff to ensure they are ordered Review percentage report, check for transferred medicines, delete duplicate orders and re[1]order previously supplied medicines if required through the pharmacy portal system Manage medicines returns, identifying those that can safely be returned and recycled and carry out safe disposal for medicines waste Ensures the maintenance and review of ward stocks, liaising with departmental managers, directorate pharmacists, medicines management technicians and the stores and distribution team Participate in the development of the service to improve patient care or increase efficiency Act as a role model for other members of the support staff To participate in evening, weekend and Bank Holiday working in keeping with the Pharmacy Directorate arrangements and the needs of the Trust Person Specification Qualifications Four GCSE or equivalent grade C or above - Essential (Must include Maths and English) NVQ Level 2 Pharmacy Services or Pharmacy equivalent - Essential Experience Demonstrable experience working in a pharmacy environment - Essential Experience of working in a team Essential Experience of working in pharmacy with Pharmacy IT systems - Essential Ability to meet deadlines whilst paying attention to detail -Essential Ability to work under own initiative and with supervision - Essential Experience of dispensing prescriptions Essential Knowledge Knowledge of hospital pharmacy supply and discharge planning - Essential Knowledge of stock control systems and storage of medication - Essential Medicines optimisation - Desirable Skills Clear and accurate verbal and written communication - Essential Numerate able to carry out simple calculations - Essential Literate able to follow verbal and written instructions accurately - Essential Good communication skills - Essential Able to prioritise tasks and manage own workload -Essential Is able to use own initiative and shows attention to detail - Essential Ability to work under pressure to meet deadlines -Essential Ability to demonstrate that can work in a methodical and accurate manner - Essential Ability to set routines and implement new working procedures - Essential Ability to work in a busy, fast paced environment - Essential Ability to use different IT applications - Essential Other Reliable and flexible to the needs of the service - Essential Self-motivated - Essential
