Job description

A great opportunity for a 12 month fixed term contract / secondment as been created to support the Trafford Macmillan Supportive & Palliative Care Team in Trafford Local Care Organisation.

The role will mainly support the Physiotherapist & Occupational Therapist in the team to support the delivery of rehabilitation and improving the quality of life to patients requiring specialist palliative care input within Trafford. The role will also support other members of the MDT.

The role will mainly be working with patients within their own homes and other community settings.

Under guidance and supervision from the Occupational Therapist (OT) / Physiotherapist (PT) / Nurse (N) / Dietitian (D) post holder will be expected to:

Enable patients to achieve their rehabilitation goals

Independently carry out visits to patients homes, deliver and fit equipment as recommended and complete specific rehabilitation tasks.

To work as an integral member of the team, participating in designated meetings and service developments.

Be responsible for designated administrative duties, general stocktaking, maintenance and storage of therapeutic equipment.

Carry out initial assessments which have been re-triaged by a qualified team member.

Assess for and prescribe a limited range of equipment
