As we have launched a new Reablement and Independence Service, we have also agreed new working patterns and you must be prepared to work as per an agreed shift schedule, including working some evenings and weekends. Your main accountabilities will be:
- To explain to residents the process of Integrated care, including how the resident will be supported but also discharged from the service. To arrange for the delivery of services to support the resident in their home, to liaise with Reablement, locality teams, equipment providers, housing services and the health hub, paying due regard to any statutory responsibilities.
- Support to evaluate this role in term of the patient outcomes, carers views, operational outcomes and use R- outcomes.
- To carry out complex O.T assessments of patients’ needs and decide how to meet the identified social, physical and psychological needs of residents. To provide this assessment in the residents own homes using all available resources, including the use of videoconferencing facilities, if required.
- To respond to a request for a LAP O.T assessment, within the agreed criteria of, urgent 2 hrs, same day and next day as agreed with the referrer. If the referral is downgraded to explain the clinical reasons for this to the referrer and provide feedback to the LAP them on outcomes of assessments and recommendations made.
- To provide an OT assessment and action plan to support patients who are deemed fit to stay at home their GP, paramedic or SCAS but who need an O.T assessment to stay at home, thus preventing admission or carer breakdown.
You will need to have:
· Ability to manage autonomously a clinical caseload of clients at a specialist level, assessing and treating clients and maintaining clinical records.
· To have recent knowledge of falls management and education, rehabilitation programs, frailty and complex care management.
· Good general knowledge of the safe installation and use of assistive equipment and adaptations.
· Demonstrate the level of experience required to work autonomously in an integrated MDT primary care health and social care setting.
· Excellent time management skills. Including the ability to work to deadlines
· Demonstrate positive attitudes and behaviours, taking a strength based approach to assessments
· Experience of working with people of diverse cultural and religious communities
· MSC/ Bsc/Diploma in Occupational therapy
· State Registration (HCPC & College of OT)
· Full Driving Licence with access to a car on a daily basis
