Permanent Contract | 23 hours per week | £13,095 to £14,952 per annum (Grade E) | Norwich

Norfolk and Waveney are on an ambitious journey to embed Discharge to Assess as part of our Ageing Well Accelerator programme. We have already made great strides since the beginning of the year and now need to take forward further rapid redesign in line with national policy and quality improvements to ensure people being discharged from the NNUH are supported to return home, access reablement opportunities and plan their ongoing care and support needs once discharged.

The Living Well Officer will provide a range of administrative and support services to Locality based HomeFirst Hubs (HFH) to support an effective and efficient hospital discharge in line with Discharge to Assess (D2A) and Network of Escalation Avoidance Team (NEAT) procedures. Collation and communication of information to enable effective discharges and crises/admission avoidance (AA) to take place.

The HFH is a co-located, collaborative, multi-agency and multi-disciplinary team for the triaging, scheduling, and case tracking of all D2A 1-3 and complex AA (NEAT) referrals for Central Norfolk citizens. The team is operational 7 days a week: Mon – Fri 08:00 – 18:00, Sat 09:00 – 17:00, Sun 10:00 – 16:00

The role will work within the HomeFirst Hub to effect safe and timely discharges for Pathways 1-3 and to support admission avoidance for North and South Norfolk and Norwich.

You will be a team player with great working knowledge and experience of using Teams, Excel, data entry and Microsoft outlook. You must be able to multitask including word processing notes whilst taking the information over the phone. You must be flexible, have a positive attitude and respect for sensitive data

Your main tasks are answering calls, checking various inboxes, taking notes from morning huddle to update excel, inputting data into a planner app. Emailing out some correspondence. Data input. Inputting data onto rotas and annual leave planner and operate in accordance with NCC and departmental practice and guidance.

You will have good listening and analytical skills, be flexible and adaptable to new ways of working and be committed to supporting people to leave hospital safely.

There are 2 part time roles available for 23 hours over 3 days. Some weekend working is essential and will be required.

This is an exciting time to join the Home First Hub as we are a developing service. We will expect committed performance from you but will offer a strong induction, the opportunity to learn new skills and a supportive working environment.

Although based at Reed House Norwich there will be some remote working each week.
