· We employ over 25 General Practitioners, with key areas of clinical excellence and a compassionate manner in order to provide the highest quality of holistic care for all patients.
· We work in partnership with a comprehensive multi-disciplinary team including Pharmacists, Advanced Nurse Practitioners and Paramedics (for pre-assessment screening and home visits).
· We have a dedicated administrative team who support the provision of a highly effective patient recall system for chronic disease management.
· We have a designated Workflow team responsible for the receipt, circulation and management of all clinical correspondence, liaising between all disciplines of staff to provide support to our clinicians.
· We enjoy the support of a full Business, HR and IT support function, led by a centralised senior leadership team with a wealth of expertise.
Due to expansion, we have a number of successful opportunities available to join our Primary Care Network. The successful candidates will be offered a flexible working pattern and generous package including up to 25 days annual leave plus bank holidays.
What do we need?
We are seeking a part time Practice Nurse, 30 – 35 hours per week. You will be part of a dynamic and innovative team who value teamwork, professional practice, mutual support and a friendly working environment.
The ideal candidate will be an RN who will work collaboratively with the whole general practice team to meet the needs of patients, whilst supporting the Lead Nurse in the delivery of policy and procedures and direction for the nursing team. The role requires you to possess high levels of professional clinical practice and will be able to deliver a range of services to patients including phlebotomy, wound care, cytology, ear irrigation and immunisations (adult and child).
What are the main duties of the role?
To be competent or express a willingness to develop the foundation skills in Practice Nursing, including child and adult immunisation, cervical cytology, sexual health, warfarin monitoring and the Long-term condition management.
Clinical Practice:
- To provide high quality nursing care that may include treatment, screening, advice, and minor therapeutic procedures.
- To assist the GP with minor surgery and Coil/Nexplanon clinics, as required.
- To advise and promote contraception where skilled to do so.
- Venepuncture, urinalysis, and sample collection in accordance with local guidelines.
- To provide safe and evidence-based wound care, liaising with and referring on to tissue viability and community leg ulcer teams as appropriate. Escalating to a GP as necessary.
- To ensure emergency equipment is maintained and up to date and to operate a system for checking expiry dates of all drugs and vaccines
- Order and be accountable for drug and vaccine stock and its rotation, liaising with other team members.
- Participate in clinical audits, as required.
- To maintain accurate patient records, use of appropriate codes, using agreed standards of data entry, templates and protocols as is required for good patient care.
Management of Long-term Conditions
· Conduct annual reviews for patients with diabetes, conducting screening for complications and reporting / referring on where appropriate.
· Utilise the Quality and Outcomes Framework (QoF) to ensure patients receive the appropriate care processes to ensure their condition is appropriately managed.
· Promote health and wellbeing, by signposting to services where necessary such as, diabetes prevention programme, smoking cessation, or weight loss services.
· To assist in the management of patients with asthma or COPD, using various diagnostic tests such as PEFR, FeNO and spirometry where trained to do so.
· Conduct annual asthma / COPD reviews, reporting poor symptom control to GP / Lead Nurse as appropriate.
· Hypertension reviews, education and implementation of 24-hour monitoring.
· Work with patients to support adherence to prescribed treatments and promoting self-management through a range of tools and health promotion.
Infection Prevention and Control
- To ensure cold chain procedures for vaccine storage are followed, maintained, and documented.
- Ensure Infection and Prevention Control (IPC) practices are maintained, effective and up to date, participating in audits as required in line with the CQC.
- To maintain the nursing and treatment rooms in good order.
Immunisation and Screening
· In line with national programmes and local guidelines, provide cervical screening to eligible women and assist in the promotion of such clinics
· To carry out routine and catch-up child immunisation clinics in line with the nation programme
· To support the annual flu campaign and other mass population vaccination programmes
· Work closely with clinical and admi teams
· Provide a comprehensive travel advice service, including health promotion, disease prevention and immunisation where appropriate
Team Working
· To support the Lead Nurse in the management and smooth running of the nursing team, assisting others where necessary.
· To act as a role model and support more junior members of the team such as fellow practice nurses, nursing associates and Health Care Assistants.
· To participate in the supervision of undergraduate nursing students
· Liaise with other multi-disciplinary teams such as those within the practice and the wider community.
Professional Development
- To maintain up to date with all mandatory training, including BLS, anaphylaxis and safe-guarding.
- To maintain annual professional registration and 3 yearly revalidation with the NMC and adhere to The Code.
- To take responsibility for own Continuing Professional Development, ensuring knowledge and clinical practice remains current and effective.
- Participation in an annual individual performance review, including taking responsibility for maintaining a record of own personal and/or professional development (PDP).
- The post holder will participate in any training and development programme implemented by the Surgery as part of this employment.
Responsibility for administration
Other duties and responsibilities to be undertaken may include any or all of the following
- Taking messages and passing on information as appropriate.
- Computer data entry/data allocation and collation, processing and recording information in accordance with national and local practice procedures.
- Initiating contact with and responding to requests from patients, other team members and associated healthcare professionals and providers.
- Helping to keep all practice areas tidy and free from obstructions and clutter.
- Helping to maintain a positive, patient focussed culture.
- Helping to maintain a positive, supportive culture across the whole of the practice team.
- Any other duties commensurate with this position.
