Clinical Practice and Quality Demonstrate competence and clinical skills in the assessment of patients in the general practice setting, ensuring care is planned in partnership with patients and all information is clearly and objectively recorded according to practice policy. To do this you will be required to: Practice in compliance with your respective code of professional conduct and scope of practice, being responsible and accountable for your decisions, actions, and omissions at this level of practice Demonstrate Minor Illness competency (either through coursework or experience) to spend up to 25% of your contracted hours working as part of the Mereside acute Duty Team in the triage, assessment and diagnosis of patients presenting on the day with minor ailments. The precise percentage is a conversation to have with you, depend on your skillset, your preferences, and organisational need. In order to progress within range this is likely to increase to 25-100% of your contracted hours Assess patients, taking into account their physical, mental and social states, alongside the impact of their environment and support available to them, to produce a person-centred plan of care Have Diploma level qualification or equivalent specialised experience in at least one of several priority clinical areas, to support the wider Mereside team (including senior nurses, Advanced Practitioners and GPs) with greater autonomy to deliver effective care to this group of patients.
Such areas include (but are not limited to) Diabetes Anticoagulation Respiratory Womens Health Cancer Care Actively contribute to implementing the quality governance systems and processes across the organisation, demonstrating receptiveness to challenge and learn, and preparedness to constructively challenge others, in doing so. This includes but is not limited to healthcare records, learning points, and significant events Be confident in, and/or willing to learn, relevant technologies to facilitate remote consultations and effective cross-site working (e.g. MS Teams, AskMyGP, SystmOne, AccuRx) Be willing to support the Nursing Team on occasion and as required in covering General Practice nursing responsibilities. Clinical Assessment and Patient Care While working within the Mereside Duty (Acute Care) Team, participate in and undertake triage of urgent care requests to ensure the smooth progress of patients through the system on a daily basis.
Work in partnership with patients, families and carers, using a range of assessment methods, your expertise, and your clinical reasoning to adopt an evidence-based approach compliant with NICE guidelines to: Assess, examine, diagnose, plan, implement and evaluate holistic treatment/interventions and care for patients presenting with an undifferentiated diagnosis Prioritise health problems and intervene appropriately to assist the patient in complex, urgent or emergency situations, including initiation of emergency care Proactively identify, diagnose and manage treatment plans for patients at risk of developing a long-term condition as appropriate Diagnose and manage both acute and chronic conditions, working with relevant prescribing clinicians to integrate both drug- and non-drug-based treatment methods into a management plan Demonstrate effective communication skills to optimise patient satisfaction while managing their expectations and supporting them to make decisions, plan care, or make positive change, including: To work with patients to ensure that individual medication regimes meet their needs and are aligned with relevant guidelines and regulations for prescribed treatments To ensure appropriate use of prescribed and over-the-counter medication, and understanding of medication regimens, side-effects and interactions When dealing with sensitive matters that may also have legal or ethical considerations, for example, safeguarding, substance abuse, sexual orientation and gender identity, racial and ethnic diversity, mental illness, disabilities, etc. To support patients to adopt health promotion strategies, and apply principles of self-care, including referring or sign-posting to other services that may support their wider well-being, e.g. District Nurses, Social Prescribers, Health & Well-being coaches, etc. Work collaboratively with your colleagues within Mereside practices and across the local health economy to manage risks and issues, and integrate effectively with health and social care providers, ensuring consistency of care across settings and practices.
Facilitation and Learning Able to recognise personal development needs and also facilitate learning for patient, carers, other staff and students. This requires you to: Critically assess, and take responsibility for, your own learning and professional development plan that reflects the breadth of the needs of practice nursing in primary care. Proactively share learning, information, and good practice insights across the practice and wider organisation to facilitate individual and team learning. Advocate for, and positively contribute to, a cohesive culture of organisational learning and collaboration both within and between teams, to ensure the best patient experience throughout their entire journey.
Build confidence and competency within the team through work-based and inter-professional learning, mentoring and role modelling. Leadership and Teamwork You are part of the Mereside practice nursing team with delegated responsibility for patient care, being accountable to the Nurse Manager. This requires you to: Contribute positively to a sense of cohesion, and foster a culture of wider collaboration, across the organisation, engaging effectively with collaboration tools such as MS Teams to improve communication and information sharing across sites and teams, including remote workers Manage at least two other members of the Nursing Team to work effectively and productively, modelling Meresides values and constantly seeking to improve ways of working both within the team and throughout the patient journey to ensure efficient and effective delivery of care Play an active role in mentoring and role-modelling for the nursing team as a whole to help them deliver better patient outcomes and continue to develop their professional competence in managing a broad range of long-term conditions and local care pathways Prioritise own workload and manage your time effectively to ensure that cases handled by the most clinically appropriate member of the team, and completed in a timely manner Work closely with the Nursing Manager and Clinical Manager, and wider clinical team as appropriate, to identify opportunities for wider service improvement initiatives in response to feedback, evaluation, and need.
