Job description
Working with the Integrated Care Area (ICA) this role is pivotal to ensuring that the business of ASC is managed at place through the Primary Care Network (PCN) Manager network. The Partnerships Manager is accountable to the Area Director and Locality Manager for the provision of good quality services to adults. This will include the assessment and review functions of operational social care staff.
The Partnerships manager will play a lead role in ensuring their local ICA Area Adult Social Care services are fit for the upcoming Adult Social Care Inspection Framework and works with system partners to continue on the journey of Health and Social Care Integration.
Working in effective partnerships with ICA colleagues from our community health provider, general practice and allied health professionals to ensure individuals within the area get access to the right support, at the right time and are supported by the right person. This will include working across both the ICA front door and Locality Services to prevent, reduce and delay the need for long term care and support where appropriate, and providing timely and proportionate assessment of need when required.
The post holder will work to ensure that the most complex people living in our communities receive the right level of care and support, that their health needs are properly identified and where appropriate funding from Integrated Care System Partnerships and NHS funding sources are identified and applied for. They develop an excellent understanding of the adult’s lived experience and are able to convey their views and feelings through high quality practice skills in assessment, planning, intervention and review. They use evidence-based practice to improve the adult’s welfare and safety, leading to improved outcomes.
The ICA Partnerships Manager will represent the ASC ICA at joint forums, including system escalation meetings, stakeholder networks and PCN networks. They will promote across the West peoples right to choose where to live and the right to choose how to live.
