We currently have a number of exciting opportunities for confident and skilled Social Workers to support us in delivering our service for adults in need of social care. Our approach is strength-based, and we aim to achieve the best outcomes - enabling people to be safe, as independent as possible and to live the lives they want.

Reporting to a Team Manager or an Assistant Team Manager, Social Workers are responsible for assessment, support planning and safeguarding vulnerable adults. You will work with service users, family carers and commissioners to ensure that support packages comprehensively meet assessed needs, whilst also being innovative and cost-effective.

Our Teams

Locality Teams

Services for older people, people with physical and sensory disabilities and people with mental health related needs are delivered by our 4, locality teams. These bring Social Workers together with NHS colleagues, supporting holistic, integrated working. The teams are based in Carshalton, Wallington, Sutton and Cheam

Hospital Team

Our social care staff within the hospital team work closely with NHS colleagues from acute and rehabilitation facilities, and with community health services, to facilitate safe, timely discharge. Their priority is to enable people to return to their own homes, utilising reablement and other services to enable them regain independence.

Requirements:

Minimum 2 years of UK post qualification experience in Adults Social Care frontline adults social care services
