Job Title: Homeless Prevention Worker
Reports to: Team Leader
Salary: Scale 6, Points 26-26, £24,744 - £26,398 per annum, pro rata
Hours of work: Full Time (37.5 hours per week)
Key Responsibilities:
- To work closely with other organisations and agencies to provide holistic multi- agency responses to housing and support needs.
- To provide a prompt and proactive, non-judgmental response to referrals to ensure engagement from the young person.
- To engage and undertake effective assessments providing, Information, Advice and Guidance to ensure young people are sign posted to the relevant services/agencies and
- assist in the delivery of an Advice and Wellbeing service to young people who may be homeless or at risk of homelessness in the area.
- To undertake appropriate background checking to ascertain level of risk and need i.e. previous landlord, parent/carer, probation etc
- To develop and maintain effective partnership arrangements with regulated and/or approved Private Landlords, Social Housing Providers and Supported Accommodation Providers through effective communication and clear understanding of roles and responsibilities.
- To carry out homelessness prevention work, including liaising with landlords, families, and others, and by applying mediation and conciliation techniques wherever appropriate.
- To report on outcomes, manage and prioritise own caseload and evaluate each case, making decisions on appropriate options, meeting deadlines, and referring to other services and outside agencies as appropriate.
- To safeguard young people and vulnerable adults at risk, through appropriate escalation and reporting to statutory services.
- To attend panel meetings and review / case conference meetings regarding young people as and when required.
- To provide advice and support to a range of stakeholders such as Service Heads, Social Workers, Youth Offending Service, drug and alcohol services, physical and mental health services, housing providers etc
- To develop and deliver training/ briefings to partnership agencies about the service and to be an ambassador for an integrated approach to working with young people to offer early help and their wider support needs at the earliest opportunity.
