Job Title: Homeless Prevention Coordinator (HRA)
Reports to: Team Leader
Salary: Scale SO1, Points 29-31, £26,137 - £27,865 per annum, pro rata
Hours of work: Full Time (37.5 hours per week)
Key Responsibilities:

To ensure that home visits and accompanying young people to appointments are
appropriate and undertaken in line with agreed lone working procedures.
- To ensure that interventions are planned, implemented, reviewed and evaluated to
and/or relief pathway duties.
- Ensure fair allocation of cases in line with service criteria and regular assessment/
- Maintain partnerships with key agencies including, voluntary sector, local authorities

Team Management Duties:
- Support teams to provide high quality support services that promote choice and
