Job summary
We have an exciting new role for post of home visiting healthcare assistant to work with South Westminster PCN member practices. We are seeking a motivated healthcare assistant to perform routine checks in line with the current PCN DES contractual requirements to housebound and vulnerable patients who are not able to come in to the practices.
Job description/responsibilities
- To carry out health screening and measurements such as BP, Height/Weight/BMI) to collect lifestyle data from patient, including smoking status, alcohol consumption and exercise.
- Diabetes at risk, and Diabetes Level 1 delivery, inclusive of all tests detailed above, goal setting, reviews of progress. Care plans for diabetics are in place and reviewed when required as per specification
- Phlebotomy clinics and handling of biological samples requested by clinician, and to ensure samples are appropriately labelled and packaged for transport
- Involvement in administration of flu vaccines & Vit B12 on the instruction of a regulated professional (PSD)
- Involvement in delivery of QOF indicators relevant to role such as Arterial Fibrillation recording and forward onto GP if High risk
- Care plans for elderly >65yrs who are on Frailty Index as Moderate or Severely frail and reviewed when required as per OOH specification
- Helping to raise awareness of health and well-being and how it can be promoted through referral to other health trainers, IAPT etc.
- Ensure collection and maintenance of statistical information required for regular and ad hoc reports and audit such as enhanced services, DES, locality plan, in addition to QOF.
- Attend and participate in Practice meetings and HCA forums as required
- Other duties as directed
Essential Qualifications
- Care Certificate
- M&K HCA in general practice (or equivalent)
- M&K Infection control (or equivalent)
- M&K Care for the elderly (or equivalent)
- Qualifications in areas of delivery (Spirometry, ECG, ABPM, Phlebotomy Health Checks, Diabetes)
- Mental Health Training
- Chaperone
- Vaccination (Flu, VitB12)
- Basic Life Support
- Willing to travel
- Safeguarding
- Be able to provide 2 satisfactory references and a satisfactory DBS check
Desirable
- Asthma and COPD training
- Knowledge of SystmOne
